Symptomtext

Patient was inadvertently administered SHINGRIX instead of Yellow fever vaccine at first. The nurse grabbed it from the fridge reconsituted (presuming it was YF) and administered SQ. Upon documenting vaccine noticed it was in fact Shingrix. Patient noified of the issue, and was informed of local reaction. Due to patient departing our area for her travel, still wished to receive her Yellow fever vaccine in the other arm. Patient does report redness in area of her arm where she received the shingrix vaccine.

Dizziness Supine position Vertigo Vision blurred

Symptomtext

About 3 minutes after yellow fever and typhoid vaccinations patient complained to nurse that she was dizzy and room spinning. Patient assisted to supine position, vital signs taken and normal. Patient A&O. Patient did not have syncopal episode and remained alert and answered questions correctly. Patient able to see up close but after observation of about 30 minutes she complained that her vision was blurry. Recommended patient to follow up in ER for evaluation or at least have a family member or friend pick her up. She refused both. Patient insisted on going home and ambulated without assistance out of the office.
