Rash papular

Symptomtext

Generalized flesh-colored papular rash within 24 hours of administration. No respiratory distress, no angioedema, no fevers. No neurologic changes. Presented to clinic on 5/5/23 for additional catch up vaccinations. These vaccines were held off for now secondary to patient presentation. Prescribed cetirizine and topical hydrocortisone for rash and will follow closely

Crying Erythema Eye swelling Injection site erythema Rash Urticaria

Symptomtext

After the shots, the baby started to cry inconsolably and after five minutes, mom called the nurse stating that the left leg is becoming very red at the site of injection and she thinks that the baby may be having an allergic reaction. Benadryl 2mL PO was administered and 2mL PO of infant Tylenol for pain. Baby was continuously high-pitched crying and erythematous hives spread all over his body. The baby's eyes became swollen and the heart rate was over 200. Within 15 minutes, IM epinephrine 0.1mg was administered, after which all symptoms resolved. The rash was gone and the baby was calm. Pt. was taken for observation to the nearest emergency room by ambulance.

Symptomtext

Patient was given a first pentacel for the visit and nurse noticed as doing paperwork that the vial had not been reconstituted as powder was still in one of the two vials so only the DTaP-IPV diluent was given. Then to correct the mistake nurse gave another dose of full pentacel DTaP-IPV-HIB to revaccinate the patient when nurse should have just corrected the initial mistake by giving a single dose of HIB. Therefore patient received and extra dose of DTaP-IPV.
