One of my new MAs was administering a flu vaccine to one of our patients. She realized that when she administered the flu shot to the patient, she had grabbed the high dose flu vaccine instead of giving her the regular dose. Patient did not appear harmed. MA notified the nurse, me, and then I notified my PA and quality department same day.
