patient found having syncopal episode after Shingrix shot. patient helped to seat by staff, responsive @ 11:20am, this RN called MD stat @ 11:21. RN noted pt breathing with grunting breath sound with small coffee ground emesis around comer of mouth, VS 143/97 HR 68 on RA 100% @ 11:23. MD stat team arrived @ 11:24am. SBAR report given to MD and RN. Care handed over. Patient was lethargic after taken over and 911 was call, Engine arrived @ 11:31am

According to hospital neurologist on 10/28: He tells me that he received his flu shot 7 days ago yesterday in (actual day determined to be 10/18/22). He received it from his primary care physician. 3 days ago he was in his normal state was even able to play an 18 hole round of golf. He went to bed the night before last at 10 PM in his normal state. He woke up yesterday morning with extreme paresthesias in his feet and eventually in his hands bilaterally as well. He went to bed at 10 PM last night and woke up at 3 this morning. When he attempted to stand up to walk to the restroom his legs did not hold him and he fell to the floor. He currently continues to have severe paresthesias of his feet and hands and he feels as if they have worked proximally as well. He tells me he feels as if his feet are in concrete blocks. He was able to lift his cell phone earlier this morning but currently he is unable to even lift his cell phone with his upper extremities. He had cognitive/speech deficits, dysarthria, developed quadriplegia and VII nerve palsy, diplopia, He was diagnosed with Guillain-Barr? syndrome secondary to flu vaccine. Treatments ongoing, but include Gabapentin, IVIG, plasmapheresis. IVIG not effective. Patient has completed 3 of 5 PLEX sessions. Patient is more awake and alert now.

Symptomtext

Patient called 10/26/22 to report that she received covid-19 bivalent, flu HD quadrivalent, and shingrix dose 1 at 9:52 am on Monday 10/24/22. She stated that Tuesday evening (she did not say what time) that she was at a friends house and felt nausea, light-headed, and her friend told her she passed out. She doesn't believe she passed out for long but isn't sure. Her friend called 911, but patient refuse to allow them to come check on her and the call was ended. The friend used an automatic BP monitor to check on patient and according to patient her blood pressure was normal. She then felt cold and chills/shakes for about an hour. The adverse reaction resolved fully by the time she notified the pharmacy 10/26/22 around 3pm. She said there was no irritation at the injection sites and was concerned about why it happened -possibly due to multiple vaccines.

Patient stated her blood pressure was elevated the rest of the day following her vaccine. Reporting a systolic blood pressure of 200. Patient also stated she was nauseated the following day but felt better and back to normal on 11/20/2022.

Symptomtext

After I administered the flu shot, patient developed a quarter size knot near injection site. Monitored patient and notified him of the reaction. Patient stated he felt fine. Gave patient Benadryl and counseled him to take it before bedtime to help with the swelling. Counseled patient on what to look for if reaction worsened. After several minutes, the swelling did not worsen so I let the patient leave. Patient said he felt fine to leave.

Symptomtext

On 10/22/2022 at approximately 2:20PM, the patient called to report local injection site pain, minor swelling around the area, and erythema extending from top of shoulder and down to about the start of the elbow. Patient reports no itchiness, no difficulty breathing, and no spreading of other individual rashes about her body. Patient was informed that erythema is not uncommon with the high dose flu shot, but that it was important to monitor her symptoms. Patient was advised to mark the furthest edges of the rash with a marker or other similar implement so as to track the rate of grown or contraction of the rash. Patient was also advised to seek immediate medical care in the event that she experienced anaphylactic symptoms. Patient was advised to consult with their PCP if the rash seems to continue to get worse, but has not worsened to the point where immediate medical care was advisable.

Symptomtext

Denies any problems with vaccines. Patient states that her provider gives her the High Dose Flu vaccine because she gets pneumonia easily and because of her health problems. I explained that we do not give it before age 65 and that is why I am making this report. She did not feel that there were any adverse effects.

Patient received Booster and flu shot by Pharmacist. After the shot, patient was then told to stay for monitoring. within minutes, patient started to have reaction to where she stated that her throat is closing and then her speak were very faint. resident was then given epi pen, 911 was called. oxygen stayed above 90%. 911 arrived and assessed her. Patient was then taken to local facility for further evaluation.

Patient was asked several times by 2 healthcare providers if she wanted a HD flu vaccination. After the vaccination was given, and patient had left. She stated that she was expecting a Shingrix vaccination. She had already received a flu vaccine at another location. She had no adverse reactions and will come in a couple weeks to get the Shingrix.
