Patient described intense pain that started in her legs and moved up her back within 5 minutes of vaccine. She didn't not have swelling of the face, neck or tongue. She stated the pain made her short of breath. I, pharmacist, called 911 while sitting with the patient. She did not appear to be having an anaphylactic reaction but Epipen was next to me. EMTs arrived and took patient's vitals. By this time, about 25 minutes after initial symptoms, patient states the pain had subsided and declined going to the hospital.

ACCORDING TO PATIENT, SHE DEVELOPED A RED-RAISED SPOT AT THE SITE OF INJECTION A DAY AFTER THE VACCINATION AND AFTER THE NEXT FEW DAYS IT STARTED ITCHING AND LITTLE BUMPS STARTED TO DEVELOP. ACCORDING TO HER, SHE CALLED AND SPOKE WITH HER NEPHROLOGIST AND WAS ADVISED TO USE SOME HYDROCORTISONE CREAM WHICH HELPS THE ITCHING BUT THE LITTLE PUMPS ARE STILL THERE. PATIENT SHOWED ME A PICTURE OF HER ARM THAT SHE TOOK AS OF MORNING OF 3/18/2023 AND SHE ALSO SHOWED ME HER ARM ON THE SAME DAY 3/18/23 EVENING AND IT LOOKED JUST LIKE WHAT SHE DESCRIBED; RAISED RED SPOT WITH PUMPS/BIG PIMPLES-LIKE.

Had tenderness in his left arm where the injection was given since it was administered/tiny pain occasionally when moving his arms; Initial information received on 28-Nov-2022 regarding an unsolicited valid non-serious case received from a consumer/non-hcp. This case involves a 75 years old male patient who had tenderness in his left arm where the injection was given since it was administered/tiny pain occasionally when moving his arms after receiving vaccine INFLUENZA QUADRIVAL A-B HIGH DOSE HV VACCINE [FLUZONE HIGH-DOSE QUADRIVALENT]. The patient's past medical history included Had vaccine before and experienced soreness for a few days (Pain in extremity). The patient's past medical treatment(s), vaccination(s) and family history were not provided. On 16-Nov-2022, the patient received 0.7ml dose of suspect INFLUENZA QUADRIVAL A-B HIGH DOSE HV VACCINE (strength, formulation was unknown, lot UJ935AA and expiry date: 30-Jun-2023) via intramuscular route in the left arm for Immunization. On 16-Nov-2022 the patient developed a non-serious event of had tenderness in his left arm where the injection was given since it was administered/tiny pain occasionally when moving his arms (vaccination site pain) (Latency: same day) following the administration of INFLUENZA QUADRIVAL A-B HIGH DOSE HV VACCINE. Action taken with QUADRIVALENT INFLUENZA VACCINE (FLUZONE HIGH-DOSE QUADRIVALENT) was not applicable. Corrective treatment: The pharmacist who administered the FLUZONE HD QIV recommended that the patient take two IBUPROFEN (IBUPROFEN) as needed throughout the day for Vaccination site pain. Outcome: Not recovered

She received the high-dose flu vaccine even though she is under the age of 65. Patient was monitored for 10 minutes after the vaccine administration and had no adverse reaction. She then left the clinic with her husband after he received his vaccine.
