Oropharyngeal pain Pallor Vomiting

Symptomtext

Pt. received MMR, Kinrix, Varicella, and flu vaccines. Within minutes she complained her throat hurt and she squated downa nd then went into a fetal type position, and vomitted (no blood). Vomit was thick and color of her breakfast cereal. Patient was talking and responsive during entire episode. No loss of consciousness. Pulse ox 98%, pulse 90-105, skin color pale.

Symptomtext

On the morning of this event i printed out the list of patient we would be seeing for the day. It tell me which vaccines the patient has or hasn't had yet. At the time of pt's appt, I misread the form and thought he was due for a flu shot. I was not aware until later that day that the flu shot I gave was expired and had been sitting out of the refridgerator for almost 24 hours. When I gave the shot to patient there was no immediate reaction.
