Symptomtext

Pt presented on 01/01/22 complaining of worsening shortness of breath. Patient states that his symptoms started on 23rd of December. 5 days later he tested positive for COVID-19. Patient continued to have worsening shortness of breath the prompted him to come to the hospital. The patient was also having diarrhea. Patient was admitted and started on Rocephin, azithromycin, Decadron, remdesivir. Patient received 2 doses of remdesivir. He did qualify for home oxygen and it was arranged. Patient also received 3 days of azithromycin and Rocephin. His procalcitonin trended down. Patient was discharged on 01/03/2022 but was unable to leave until 01/04/2022 because of oxygen. He continued improving post hospitalization.

brought to the emergency department on 2/6/2022 with chief complaints of difficulty breathing required BiPAP in the ER. Cough/shortness of breath for past week. Patient on prednisone and doxycycline from January 29th. 8 Days ago. Patient apparently tested negative for COVID-19 at that time. Repeat COVID-19 screen in the ER 2/6 positive. In the ER patient hypertensive tachycardic tachypneic hypoxic blood pressure 152/110 heart rate 137 respiratory rate 28 87% saturation on room air. Drinks alcohol 4-6 beers twice per week. CTA negative for PE, no pulmonary infiltrate No acute chest abnormalities Patient required BiPAP, received respiratory treatment IV Solu-Medrol magnesium sulfate received morphine ketamine in the emergency department. Labs radiology studies reviewed Admitted to moderate care floor. Pulmonary consulted. Patient placed on IV steroids, Levaquin, respiratory treatment.See H&P for all the details regarding admission.. 2/7- Respiratory status improved. Patient taken off BiPAP,Switched to nasal cannula. Reported having cough with brown sputum. Sputum cultures no growth to date. 2/8- Patient weaned off oxygen on room air this morning. Did not require BiPAP. Afebrile. Respiratory status stable. 2/9- Patient feels well and wants to go home. Patient denies any shortness of breath this morning. No chest pain no abdominal pain nausea or vomiting. Minimal cough. Patient seen by Pulmonary this morning. Patient received 3 days of antibiotics, discontinued today. Advised to continue Decadron for 7 more days. Cleared patient for discharge home.
