7/26/22 Vaccine received in 1600 hour. 2000 Emesis. 2400 Pt found w labored breathing and limp per mother of child. Pt roused w vigorous rubbing be parent. EMS called and assessed pt. Pt stable and no transport to hospital determined to be necessary. 7/27/22 Mother of child reports pt improved today. Appetite seems slightly less than normal. Pt is breast feeding. No hives. No wheeze. No injection site reaction.
