Patient had requested COVID 19; however; vaccine was incorrectly entered into computer system and pharmacist failed to notice the mistake. Patient had already received a flu vaccine at this location on 9/19/22 and this was a second dose of same. After discovering the mistake, the COVID 19 Pfizer Bivalent was given to the patient. Pharmacist discussed implications of receiving a second flu shot and patient was satisfied with the discussion.
