On the morning of 3/21/2023, I noticed that I had a sore throat. The sore throat lasted for a couple of days, and on 3/22/2023, I started to also feel tired. In the early hours of 3/23/2023, I woke up with an extremely sore throat. At 2:30 AM, I took a lateral flow COVID-19 test and got a positive result. On 3/23/2023, I had a telehealth visit with my doctor, who prescribed me Paxlovid. She also told me that I could take OTC medicines like Advil and Tylenol to manage discomfort. I began taking the Paxlovid on 3/24/2023. I soon started to feel better afterwards, though I continued to test positive. On 3/28/2023, I finished the Paxlovid. On 3/29/2023, I tested negative for COVID-19. After all of that, I continued to have issues with breathing and coughing for about three weeks afterwards. I ended up using my inhaler about two or three times during this period.

Bronchospasm Dyspnoea Nausea Urticaria

Symptomtext

Pt received a flu shot at approx 9 am this morning. At 1330 PT called stepmother to bring him some nausea medication. At school he had O2 sats 86-90% and the school nurse asked that pt be brought into be seen. Upon rooming, pt appears to be working at breathing with deep, intentional breaths. He is able to speak in full sentences but states that he feels short of breath. Per provider, patient had bronchospasm and O2 saturation of 90% at that time. Provider directed the RN to administer Benadryl and to titrated O2 to 94% and above. Patient could maintain his O2 above 94 for 5 minutes. At that time, provider recommended pt go to the ER for management of this reaction (possibly to an influenza vaccine) as we do not have resources for monitoring the reaction as severe as this. Patient also had hives to the armpits bilaterally.

Symptomtext

Patient informed me that she had previously fainted due to vaccination. I told her that she would have to remain seated for a minimum of 15 minutes after vaccination in the vaccination area and be monitored closely. She agreed and decided to go ahead with the injection. I cleaned her arm and placed a barrier bandage on her arm then injected a dose of Fluzone Quad vaccine into her left arm. Within 20 seconds she had lost color in her face and asked to lie down. She was turned on her left side across the chairs by myself and her boyfriend and then both arms and both legs and her neck became very stiff and patient was unresponsive to voice and touch stimulation for about 15 seconds. After the stiffness occurred, her boyfriend called emergency services and Ambulance service was dispatched to the store about 10 minutes later. The EMT took patient's vital signs (pulse, blood pressure) and also her blood sugar. Patient asked for a cold pack because she said it sometimes helps her after this type of reaction and she was provided an instant cold pack by the EMT. The EMT recommended that patient be transported to the hospital by ambulance or private vehicle due to low blood pressure (93/64 and 110/64 ) to be carefully watched until full recovery. Patient declined to be transported and left the store with her boyfriend about 12:15 PM.

Pt reported improvement around 9:25am, orthostatics obtained - WNL. Geri-Lanta administered for nausea. Pt ambulated safely from clinic at 09:45.

Pfizer vaccine bivalent booster was order and administer by medical assistant. at around 1600 another MA was getting ready to work on another family and notice that vaccine vial was label with date 1/9/23 at 0909. Medical assistant immediately called LVN supervisor to huddle and determine if in fact vial was expire. on investigation it was determine that the vial was originally puncture on 1/9/23 at 0909 and it was passed the 12hrs. Provider was notified, and mother was notified. attempt to call Pfizer for guidance but office was close. we will call on 1/11/23
