Symptomtext

Patient received COVID Pfizer monovalent vaccine along with Fluzone HD vaccine at a routine appointment on 9/21/23. On the morning of 9/24/23 he began noticing symptoms of gait disturbance and generalized weakness. He presented to an urgent care on 9/25/23 with continued symptoms, worse balance and new speech difficulties with difficulty finding words and slight slurred speech. Urgent care sent him to Emergency Medical Center where he was assessed and found to ha ve and intraparenchymal hemorrhage in the left basal ganglia on CT. MRI brain confirmed a left acute thalamic intraparenchymal hematoma.

Symptomtext

Pt reports LA began hurting the evening of injection 10/16/2023. She calls stating that arm is still hurting as of today 11/20/2023. She reports that arm is very sore and has a knot at injection site. She states that LA has slight bruising, and she sometimes experiences pain that will shoot down her arm into wrist.
