Symptomtext

Patient states that she normally keeps a journal where she writes up her normal daily activities. She says that after the vaccine was given, when she got in the taxi to return home, she started writing what she calls "insane" things in her journal, things she described as jibber jabber and random scribbles. She says that during that evening she went into a diabetic coma. Her daughter found her and took her to the hospital. Her blood sugar had gotten as low as 32.

On Oct. 18, 2023 I received the Covid vaccine in my left arm and the flu vaccine in my right arm. That evening both of my arms became very sore and I developed a low fever - variable, but always below 101F. I was unable to fall asleep when I went to bed. I had to frequently get up urinate. One time when I went to the bathroom I lost consciousness and fell to the floor. I almost immediately came to and stood up, but felt queasy and unwell. I then passed out once again. When I came to, I was wet with sweat and unable to get up, with my whole body now feeling sore. My wife helped me up and into bed and I almost immediately fell asleep. I remained with fluctuating low fever and body pains for about 3 to 4 days afterwards. I then felt better, but now there was a swollen, discrete mass on the left side of my face. Over the course of 2 to 3 days, the mass moved down and into the left side of my lower lip and then to the right side of my lip, and then disappeared by the next day. (After one of my previous Moderna Covid booster shots, I developed a similar mass in the inside of my left cheek (the side of the body I received the injection), which disappeared in a day or two.)) None of these ?lumps? caused any pain or discomfort - just looked strange. I have recovered from these side effects, but still, a few weeks after, have pain in my lower back. Perhaps I was injured in the the night time falls, but I was not aware of having these specific back pains right after the falls, being then just ?sore all over.?

Symptomtext

Patient presented to ED with generalized weakness 1 day after getting her flu shot. Per patient and daughters statements, patient was fine prior to flu vaccination and able to ambulate with a cane. The next day, patient had generalized weakness and was unable to move out of her bed without assistance. Denies fever, chills, chest pain, palpitations, shortness of breath, nausea, vomiting, abdominal pain, difficulty urinating, falls, loss of consciousness. In the ED, patient AAO x 4, in no acute distress. Patient states she is fine except for her generalized weakness.

Symptomtext

Patient had three vaccines and reported difficulty breathing and chest pressure shortly after. Patient had supplemental oxygen that she was using . Patient was instructed to use the supplemental oxygen for a short time and without definite improvement 911 was called and patient was evaluated by first responders and after the assessed she was not having a cardiac event she was sent home in her own vehicle. Patient reported continued difficulty breathing after returning home and being extremely cold. Patient went to emergency room. Pharmacy received prescription for z-pak 250mg, prednisone 50mg x5days, and ipratropium/albuterol nebulizing solution

Pt states that two weeks ago she got the flu shot and about two days later she has had pain and black and blue bruise from the elbow to the shoulder on the right side. She states that she has a lump where the injection site is. She states that she is trying not to move it. She states that she is using asper cream to help with the pain. Pt states that she is a little nauseous and dizzy. She states that she has been using ice, heating pad. Pt states that the lump never grew after it showed up two days after the shot.

Symptomtext

Clinical course at hospital indicative of the development of sepsis and TTP-HUS (thrombotic thrombocytopenia purpura with hemolytic uremic syndrome). Pt received dose 5 booster shot on 10/15/2023 (LOT #GJ6665). Dose number 6 was given on 10/21/2023. Pts husband reported pt started feeling unwell the night of 10/21/23. Pt presented to the ED on 10/25/23 C/O abdominal pain and rectal bleeding. Pt was Dx with Colitis and D/C with Bactrim PO ABX. Per Pts husband pt continued to deteriorate at home. Pts husband called 911 on the morning of 10/29/2023 stating that pt did not recognize him and was nearly unresponsive. Upon admission to the hospital Pt was in renal failure and had no purposeful neurologic movement. Pts husband moved to comfort care on 10/30/2023 when PT was rapidly deteriorating and was moving towards needing intubation and ICU level of care. The need for Plasmapheresis was being discussed when she moved towards comfort care. Final diagnosis was not achieved because all of the diagnostic labwork was not able to be obtained. however what was obtained was highly abnormal and leaning towards TTP-HUS. Source of sepsis was unknown at the time she was moved to comfort care. The majority of labwork was hard to obtain because it was all coming back hemolyzed.
