Symptomtext

Patient stated she had intense lower back pain that spread to her legs. She then noticed her left leg started tingling and became numb. This then spread to her right leg and she became unable to walk. This lasted until the next day and then began improving. She did not seek any medical attention. By 48 hours all symptoms had improved. By 96 hours she said she was back to 100%.

Symptomtext

Patient received a number of vaccinations as a part of her routine 4 year old well visit on 11/29/2023. Her mother called on 12/1/2023 stating the site on the patient's right thigh, where she had received Quadracel and influenza vaccinations was red and swollen and was increased from the day prior. Patient was started on oral antibiotics (clindamycin) and given an appointment to be seen in clinic on 12/1/2023, the next available appointment. She was evaulated on 12/1/2023 in our clinic with improvement of the erythema and swelling.

Symptomtext

Vaccination was administered on 11/16/2023. On 11/17/2023 she was seen in our clinic complaining of redness, swelling, and pruritis at site of injection. The patient was treated with topical steroids and oral benadryl. That evening, her mother called stating that there was an increasing area of redness at the injection site and oral antibiotics (cephalexin) were prescribed. Patient returned to clinic on the next working day (11/20/2023) and was evaluated. The physician noted that the cellulitis appeared clinically stable with improving physical signs.

When entering vaccine into patient record the following day, it was noted patient had two records which showed she had received HD flu vaccination August 26, 2023 from another facility. No adverse reaction known. Unable to reach by phone, left message. Records merged.
