Symptomtext

Extreme pain in back, left shoulder, neck to the point of severely restricted movement & pain when taking full breaths. Sunday evening 6/6/21 around 8:00PM I thought I was experiencing heartburn & took 20mg of famotadine which didn't alleviate my symptoms. Overnight the pain in my chest moved to my neck, back, & left shoulder. I called the hotline & was advised to go to the ER to be checked for a heart problem. The ER staff ran tests & discovered nothing wrong with my heart. I was prescribed 500mg Acetaminophen & 40mg Pantoprazole. The Acetaminophen helped with the muscle pain & as the muscle pain eased, so did the pain that came with breathing so I think I never had heartburn, just bizarrely extreme muscle pain.

10:58AM Pt complains of heart rate "going too fast." Pt denies chest pain, sob, dizziness or nausea. Vitals signs were obtained and pt was given water and juice. Pt reports "I feel fine except for my heart rate going fast." Aid (EMS) was called and attended to pt. Pt was cleared by EMS and was safe for pt to go home with her husband driving. Pt reported "I feel okay now." 11:15AM Pt reported a second time of feeling her heart rate "going fast again." Pt denies chest pain, sob, dizziness or nausea. Aid was called and attended the pt. Pt remained alert and oriented, speech and gait at baseline. Pt skin warm and dry. Aid collected vital signs. Aid advised pt to seek medical attention if symptoms re-occur. Pt reports "feeling better" and agrees to seek medical attention if previous symptoms return. Pt driven home by her husband.

Symptomtext

Pt arrived to post observation area post 2nd dose of Moderna. Pt reports received this injection in her L arm and now reporting L arm pins and needles feeling radiating up and down from injection site to fingertips and L side of face. Pt states after 15 min, sx resolved in arm but was now in forehead. After another 15 min, Pt reports all symptoms resolving and no other sym, no resp symptoms, no itching or troubles breathing. Pt states similar episode from first vaccine. Pt states then reporting tingling in lips. EMTs called to evaluate. EMTs evaluate Pt, obtained vitals. Pt denied swelling, SOB, throat/tongue. Per evaluation by EMTs, client and husband were discharged by EMTs. Client feels comfortable to go home with husband and husband is driving. Pt states symptoms resolving. VS per EMT at 1415 BP 148/78 HR 82

Symptomtext

Adverse event: PT abruptly felt lightheaded approximately 14 minutes after receiving Moderna COVID vaccine at 1235. PT lowered herself to the floor to lay on her left side. PT presented with pallor and diaphoresis. RN responding called for aid at 1249, aid arrived at 1251. SPO2 was 97% on RA. PT reported a history of syncope, especially after injections, denies significant medical history and was unsure of baseline BP. EMTs prompted PT to sit in chair, PT was able to rise without difficulty, but continued to c/o 'lightheadedness'. EMTs provided education on vaso-vagal response, provided juice and water to PT. PT reported feeling "stable, but a little warm" and then reported increasing nausea. PT provided emesis bag and chose to lay down once again, reporting that she felt better laying down. At 1304, PT reported that her nausea was resolved, and that she felt 'normal'. Denied vision changes, vertigo when moving head side to side. Color improved. EMTs medically cleared PT at 1313 to be driven home by husband. Treatment: Juice and water were provided, EMTs assessed/treated (Vitals at 1250: Bp 100/P laying, HR 106, SPO2 97% RA. 1255: BP 100/P sitting, HR 92, SPO2 99% RA. 1303: BP 96/52 laying. 1308: BP 102/60 sitting). Pupil check at 1300 WNL, PT declined CBG check. Education provided on s/s to watch for, when to seek medical care provided by RN. Outcome: PT was medically cleared by EMTs at 1313, driven home in private vehicle by husband. HCP Responding: RN, EMT, EMT, RN

Symptomtext

The night I received the second dose, probably after 11pm, I had body aches, chills, headache, probably a fever. In the morning, still feeling poorly, I took ibuprofen, which helped ease the symptoms. When that wore off, around 11 am on 05/18/2021 the body aches and chills returned. I checked my temperature, and it was 102.3. Later that evening, I'm not sure what time, my arm started itching at the injection site. The fever and body aches continued overnight. On 05/19/2021 I woke up with the fever gone but body still achey, plus sensitive skin. The injection site is still itchy, is slightly swollen, and has a large red patch around it.

Symptomtext

Client presented to our clinic for his 2nd dose of Moderna vaccine. His wife called the call center to report the following: "His arm is immobile ? he?s not paralyzed but paralyzed by pain. Cannot change shirt, has never had pain like this with a vaccine and noticed something was wrong immediately. 'He could feel the liquid going in.' Fine with the first dose, arm was tender. No pain, swelling, redness, tingling with this vaccine. Very isolated, no unusual s/sx. Trying Motrin/Ice Pack. Didn?t know if he should be seen ASAP or wait it out." According to the caller, the vaccinator told the patient: "Pain at the injection site is normal after a vaccine, but if your husband believes the pain is unusual or concerning to him, he should follow-up with a PCP/provider." The call center representative told the patient the following: Suggested contacting 24/7 nurse hotline, as they can provide more assessment than we are able at this hotline and provide additional recommendations. Caller endorsed understanding but requested that we f/up with site to alert the nurse that she's administering the vaccine in the wrong spot. She said specifically that the site was much higher than the standard approx of 3 finger breadths. Call center note/addendum: Contacted caller and spoke with patient directly. Patient confirmed that information provided by his spouse was accurate and also stated "Caller noticed more pain than with a normal shot, but not a sharp pain. Described as a deep pain and like "fluid was distributing throughout the shoulder as it was going in." Patient has spoken with 24/7 nurse hotline and instructed to wait a few days and see if the pain resolves because there isn't much that can be done. Encouraged caller to check in with provider if issue doesn't resolve. Caller gave permission for this information to be shared with the vax site and for VAERS report to be completed. Explained that site wanted to assure event is documented in the event that pain doesn't subside, endorsed understanding.

Client had concerns of dizziness, throat tightness, lightheadedness, dry mouth. Onsite EMTs called to the patient. Vitals and blood sugar obtained. Client was given water and allowed to lie on back to rest. Client reported a decrease in throat tightness, still felt dizzy. Client denies any history of anxiety and panic attacks. EMTs performed orthostatics. Client's wife was called to pick the client up. Client states he did not drink water today. Denies SOB or chest tightness. Client states feeling better. Client's wife took the client home and a handwritten note was given to the client excusing him from work. Client left the space.
