into residents to assess respiratory status at 2pm. Noted respiration's have changed since previous assessment. Respiration's shallow at the rate of 12 per minute. Skin color dusky in color. This nurse placed call to POA to update in change of condition. Per POA he will notify family and be into facility. This nurse sat with resident during this time. Respiration's have ceased at 2:21pm. No BP noted. Heart rate assess. TOD 2:21pm. this is the note by the nurse. Note: This was the second booster.
