Redness, swelling, warmth to right deltoid injection site. Dr came in to see patient. Cephalexin and Benadryl prescribed to local pharmacy. Return 2 days for reevaluation.

This is a vaccine administration error. The patient was given ProQuad (MMRV) rather than the intended MMR vaccine. This was given concurrently with other intended vaccinations today including Varicella vaccine (Varivax), Hepatitis B vaccine (Engerix B), and Tdap (Boostrix).

Symptomtext

Pt is 15 years old and was given ProQuad. This was her second dose of MMR and first dose of Varicella. Pt had no adverse reaction to this vaccine. Informed parents of the error immediately and explained that the patient did not need to be revaccinated according to CDC guidance.
