Symptomtext

Patient received Moderna vaccine (1st dose) at 4:41 pm and was advised to stay in observation for 30 minutes due to prior severe reaction to a medication. At 4:50 pm, patient c/o sweating and tachycardia. Patient was offered and accepted water and VS were taken with BP162/98, HR 113, and O2 Sat of 100%. Patient admitted to having history of anxiety. Patient was instructed to take slow deep breaths. At 5:00 pm, BP was 138/89, HR 94, and O2 sat of 100%. Patient stated feeling better and stated she was comfortable to go home. After 30 minute observation period, patient was advised to call 911 if symptoms got worse and if she developed any other severe reactions.
