BLOOD CLOT; HEART ATTACK; This spontaneous report received from a health care professional (nurse) concerned a 67 year old female. The patient's weight and height were not reported. the patient's past medical history included blood clot, and heart attack. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: unknown, expiry-unknown) dose was not reported, 1 total administered on 08-MAR-2021 for prophylactic vaccination. The batch number was not reported and has been requested. No concomitant medications were reported. On an unspecified date in MAR-2021, the patient experienced blood clot/heart attack. On 26-MAR-2021 the patient died from blood clot, and heart attack. It was unspecified if an autopsy was performed. The action taken with covid-19 vaccine ad26.cov2.s was not applicable. The outcome of blood clot and heart attack was fatal. This report was serious (Death).; Sender's Comments: V0: This 67 year-old female with history of blood clot and heart attack experienced fatal blood clot and heart attack 18 days after receiving the Janssen Covid-19 vaccine, No further information regarding medical history, concomitant medications, social history, diagnostic test results, or other data was available. It is unknown if an autopsy was performed. Based on the limited information which includes past history of blood clot and heart attack, the fatal events are inconsistent with the causal association to immunization, per the WHO causality classification for adverse events following immunization. I took the Johnson and Johnson vaccine approximately 1 week after it was available in my area ,through facility. I was fine the day of the shot and next day. But the 3rd day, I woke up at 5:00am feeling terrible: extreme weakness, difficulty breathing, chest pain, elevated heart rate (110-120/min), decreased O2 sat (pulse oximeter in the 80s), and mild difficulty with thinking and concentration. My doctor told me to go to the ER. After about 6 hours in which I was screened for heart attack and other differentials, and I was on IV fluid, and I think they gave me Tylenol, I was stable enough to send home. I recovered slowly over the next 3 days. About 10 days later, I had a flight into health -- all my COVID-19 long haul symptoms cleared up -- and I have stayed healthy since.

No concomitant medications were reported. On 12-MAR-2021, the subject experienced coughing. On 12-MAR-2021, the subject experienced sore throat. Laboratory data included: COVID-19 PCR test (NR: not provided) Negative, and SARS-CoV-2 rapid diagnostic test (NR: not provided) Negative. Treatment medications (dates unspecified) included: guaifenesin, and ibuprofen. The action taken with covid-19 vaccine ad26.cov2.s was not applicable. Three hours after injection I fell fast asleep. 7 hours after injection I had mild body aches and chills that lasted about an hour. I have had a dull headache for about 15 hours. I ended up sleeping for 14 hours
