Narrative: 101 year old male admitted on 3/25/22 with dyspnea on exertion. He had previously tested positive for COVID on 3/11/2022 and received monoclonal antibodies at an outside institution, unclear where. He was found to have NSTEMI on admission. He had been vaccinated x 3, Pfizer in the community (1/7/2021, 1/28/2021, and 10/6/2021). He did receive dexamethasone during admission. Date of death was 4/1 which was attributed to COVID-19, NSTEMI, Lung mass, Heart failure with reduced ejection fraction and Peripheral arterial disease on documentation of death.

D-dimer, acute pulm edema and acute on chronic renal insufficiency. Dcd to home. Six hrs later, readmitted w/ worsening multifocal airspace opacities, enlarged cardiac silhouette, sob, cough. No PE on CXR. Recd O2, cefepime, remdesivir, vanco, Lasix, heparin, rivaroxaban, dexamethasone, tocilizumab. On 3/8/2021, pt had onset R weakness, CT w/ distal R MZ occlusion, Intubated for decline. Not TPA candidate. Per neuro, CVA r/t either a fib hx or hypercoagulability r/t covid. Pt died.

I received the Pfizer Booster shot on 11/12. I had no prior symptom's with prior shots. I had knee replacement surgery one month earlier (10/11/21) . I got the booster shot on 11/12. On 11/19, I broke our with rash/hives underarms, stomach. back, buttocks. Also had 2-3 short duration anaphylactic/ throat tightening events on 11/19. I was given Prednisone, 10Mg to clear the rash.

Shortly after receiving the vaccine, the patient fainted onto the ground. She was throwing up and very pale, but conscious and sitting on a chair. Emergency services responded and paramedics checked in on her - her vital signs were normal. She opted to return home with her father and not go to the hospital.

Within minutes of shot, my son had a seizure. He complained of tingling in left arm, then he couldn?t hear anything and had blurred vision, and said he had an extreme headache. Within moments he began convulsions that became stronger and his whole body was shaking. This lasted for about 45 seconds. He stopped shaking and began to wake up. He was groggy and confused and said he didn?t remember anything, but the pain in his head subsided. 911 was called and they took him to the hospital to check his vitals. They could not be sure what brought on Seizure. Suggested Vagol Response. We went home. That evening he felt tingles and needles in body, we thought side affect of vaccine. Today, he was in extreme pain from pins and needles sensations throughout his body.

Bell's Palsy recurrence; This is a spontaneous report from a contactable consumer (Patient). A 25-years-old female patient received first dose of BNT162B2 (PFIZER-BIONTECH COVID-19 VACCINE, solution for injection, Lot number: 59267-1000-01) via unspecified route administered on left arm on 29Mar2021 at 17:45 as single dose for COVID-19 immunization. Medical history included facial paralysis two times before once in 2013 and in 2015. Concomitant medications included Zoloft 200. On 06Apr2021 at 20:00 pm, the patient experienced bell's palsy recurrence. It was reported that the patient developed recurrent symptoms of bell's palsy a week & one day out from the vaccine. The treatment for the event included Prednisone. The patient had no other vaccine in four weeks. The patient was not tested for covid post vaccination. There were no known allergies. The outcome of the event was unknown.

had a sudden severe sugar drop to 38; close to passing out; This is a spontaneous report from a contactable consumer (Patient). A 73-year-old male patient received BNT162B2 (PFIZER-BIONTECH COVID-19 VACCINE, Solution for injection, Lot number: 59267-1000-01), via unknown route of administration on left arm on 05Feb2021, at 17:00 at the age of 73 years old at a single dose for COVID-19 immunization administered at Pharmacy or drug Store. Medical history included hypoglycemia. Concomitant medications included diltiazem, lansoprazole, cimetidine, acarbose and omeprazole (ZOL). Past drug event included allergy to doxycyclene. The patient did not receive any other vaccines within 4 weeks prior to the COVID vaccine. The patient received other medications within 2 weeks of vaccination. The patient stated, "I have hypoglycaemia", "On 05Feb2021, about 4 hrs after vaccine I had a sudden severe sugar drop to 38. No warning signs like I usually have. Followed normal process of bringing sugar back up with Glucose tablets, boost and protein bar. Was close to passing out and just wanted to make someone aware that this could happen. I did call and talk to the pharmacist about it". The patient assessed the events as non-serious. The patient was not diagnosed with COVID-19 prior to vaccination. Since the vaccination, patient was not tested for COVID-19. The patient recovered from the events on an unspecified date in Feb2021.

Within 10 minutes I felt lightheaded. Went back to the pharmacy and they had me sit down. My eyes were dilated and became pale. Then within an hour I couldn?t speak or move. Signs much like a stroke. At the hospital I began to speak about 2hrs after entering hospital. Doctors ran all kinds of tests everything came out ok. But my speech and head still are not normal.

Medical history and concomitant medications were not reported. Prior to the vaccination, it was unknown whether the patient was not diagnosed with COVID-19. The patient did not receive any vaccination within four weeks prior to the COVID vaccine. On 20Aug2021 at 12:45, the patient experienced intense, migraine like headache that improved after 4 hours to a dull headache. The patient felt really hot even under a ceiling fan and in an air-conditioned room and chills developed about hour after the headache began. The events did not result in doctor or other healthcare professional office/clinic visit, emergency room/department or urgent care. Therapeutic measures were not taken as a result of the events. Since the vaccination, the patient had not been tested for COVID-19. The clinical outcome of the events intense, migraine like headache that improved after 4 hours to a dull headache, felt really hot and chills was resolving at the time of this report. The patient experienced Light headed, Chest pain and trouble breathing on 12Jun2021 11:00 with outcome of recovered with sequelae. I experienced what I believe to be atrial fibrillation (irregularly irregular pulse, intermittent pounding in chest, SOB with exertion, and intermittent dizziness and diaphoresis). Since I have had atrial fibrillation twice in the past with influenza illness, I did what I have been instructed to do in the past in case of recurrence. I took Carvedilol 6.25 mg. Five hours later, after going to bed, my symptoms reduced to an irregular heartbeat (but not what I believed to be atrial fibrillation) with mild intermittent dizziness when standing. I repeated the Carvedilol dose 12 hours after the first. My heartbeat was regular 16 hours after the episode

Hurt, ache all over body - weakness, low energy to get around walking, 1st night threw up arm hurt into back of shoulder 1.5" round pink at injection site - which seemed higher up on arm than usual...closer to shoulder Got booster on Thursday at 1:00 p.m...fine until early hours of next day with throwing up and painful aches....then sleep on and off all next 3 days with body aches all over. Little appetite or energy to make any food...but then mostly only wanted fruit. Today Tuesday, 4th day after feeling more like self...ache arm, some weakness.

Dizzy; by the time I got up on tue. I had the shakes,; Nausea; when I tried to eat I threw up,; can feel my lymph nodes are slightly swollen,; as well as Joint pain,; Chills; Muscle pain; One day after the shot my arm felt like I had just received a tetnis shot. I got the shot 17 apr a sat. by monday I was feeling weak; Slept for 24 hours; Have eat on once since Sunday; Weight has dropped from 185 to 172.; This is a spontaneous report from a contactable consumer, the patient. A 59-year-old male patient received the second dose of BNT162b2 (PFIZER-BIONTECH COVID-19 mRNA VACCINE, lot number: 59267-1000-01) via an unspecified route of administration in the left arm on 17Apr2021 at 09:15 (at the age of 59-year-old) as a single dose for COVID-19 immunisation. Medical history included recovery from heroin addiction from past 26 months and allergy to penicillin and its relatives. Prior to the vaccination, the patient was not diagnosed with COVID-19. The patient did not receive any other vaccines within four weeks prior to the vaccination. Concomitant medications included clindamycin (MANUFACTURER UNKNOWN) for unknown indication on unknown date and unknown if ongoing. On 19Apr2021, one day after the shot, the patient arm felt like receiving a tetanus shot and weak, so he layed down and slept for 24 hours. On 20Apr2021, the next day, by the time the patient got up experienced shakes, was dizzy, had nausea, when he tried to eat he threw up, lymph nodes were slightly swollen, as well as joint pain, chills, and muscle pain. On 21Apr2021, on the next day, it was worse. On 22Apr2021, after a day, the patient felt slightly better. The patient reported that he was in contact with doctor and was told to rest, take tylenol and if symptoms get worsened then to go to the emergency room. The patient was resting, but have eaten only once since 18Apr2021, the day after vaccine. The patient weight had dropped from 185 to 172 (unspecified units). The events resulted in doctor or other healthcare professional office/clinic visit. Therapeutic measures were not taken as a result of the reported events. Since the vaccination, the patient had not been tested for COVID-19. The clinical outcome of the events arm felt like had just received a tetanus shot and felt weak, layed down and slept for 24 hours, shakes, was dizzy, had nausea, threw up, lymph nodes slightly swollen, joint pain, chills, muscle pain and weight dropped from 185 to 172 (unspecified units) was resolved with sequelae on an unknown date in Apr2021. No follow-up attempts are needed. No further information is expected.

A 41-year-old female patient received BNT162B2 (PFIZER-BIONTECH COVID-19 VACCINE) via an unspecified route of administration, administered in Arm Left on 11Aug2021 17:00 (lot number: 59267-1000-01; expiry date: unknown) as dose 1, single, for COVID-19 immunization. The patient had no relevant medical history. The patient had no known allergies. Concomitant medications included fluoxetine hydrochloride (PROZAC) taken for an unspecified indication, start and stop date were not reported. The patient is not pregnant at the time of vaccination. The patient was not diagnosed with COVID prior vaccination. The patient did not receive other vaccines within four weeks prior to COVID vaccination. The vaccine was administered in a pharmacy/drug store. On 12Aug2021, at 3:00 AM, the patient experienced chest pain, headache, ear ache, nausea, tiredness, sweating, chills, major night sweats, terrible nightmares, body feels like someone has beat her with a bat, extremely tired and when she lays down to close her eyes, she had hallucinations. The patient did not receive treatment for the events. The patient has not been tested for COVID-19 since vaccination. Outcome of the events was not recovered. .; Sender's Comments: Based on the information in the case report and a plausible temporal relationship, a possible causal relationship between the events hallucination and suspect drug BNT162B2 cannot be excluded The impact of this report on the benefit/risk profile of the Pfizer product is evaluated as part of Pfizer procedures for safety evaluation, including the review and analysis of aggregate data for adverse events. Any safety concern identified as part of this review, as well as any appropriate action in response, will be promptly notified to regulatory authorities, Ethics Committees, and Investigators, as appropriate.

The patient previously took sulfamethoxazole and experienced drug hypersensitivity, bnt162b2 for covid-19 immunisation on 01Jul2021 00:00 in the left arm and experienced headache, neck pain and malaise. The second dose of vaccine was administered in the pharmacy. There were no other medications the patient received within 2 weeks of vaccination. On 22Jul2021 21:30, the patient experienced was awful/felt very nauseous, the second shot was way worse, running a fever/ fever was over 102.1 ((because that was taken after vomiting, brushing my teeth, and drinking cold water), aching/her whole body ached, head hurting/ head felt like it was going to explode, then on the same date about 22:30, she started vomiting and continued vomiting all night long/around 9:30 am finally quit vomiting but continued to feel nauseous. On 22Jul2021, the patient underwent lab tests and procedures which included body temperature was over 102.1. The patient did not receive treatment for the adverse events. The patient was not diagnosed with covid-19 prior to vaccination nor was tested for covi-19 since the vaccination. The outcome of vomiting was recovered on 23Jul2021 09:30 and all the other events were not recovered. The device date was 23Jul2021. Follow-up attempts are completed. It is nonexistent; pain where my middle finger was almost amputated when I was a child; nursing four month old; injection site pain; tiredness; headache/headache feels horrible /head throbs all the time/head hurts; nausea; my controlled melasma started to move and grow on my face; This is a spontaneous report from a contactable consumer (Patient). A 39-years-old non-pregnant female patient received BNT162B2 (PFIZER-BIONTECH COVID-19 mRNA VACCINE; Solution for injection; Batch/Lot Number: 59267100001), via an unspecified route of administration, administered in Arm Left on 07Jul2021 (at the age of 39 year) as dose number unknown, single for COVID-19 immunisation at Pharmacy or Drug Store. Medical history included chloasma, limb injury (middle finger was almost amputated when patient was a child). Patient had known allergies: Walnuts, pecans, tree nuts, cat dander, gluten and too many things. Concomitant medications included cetirizine hydrochloride (ZYRTEC [CETIRIZINE HYDROCHLORIDE]). The patient did not receive any other vaccines within 4 weeks prior to the COVID vaccine. Prior to vaccination, the patient was not diagnosed with COVID-19. Since the vaccination, the patient has not been tested for COVID-19. The patient received Zyrtec within 2 weeks of vaccination. On 07Jul2021, 1st day, patient experienced injection site pain, tiredness, headache, nausea. On 08Jul2021, 2nd day patient experienced muscle & joint pain, eye pain, sinus pressure, cough, fever 100, feeling unwell, and patient nursing four month old would not take patient breast and also reported that normally patient period has been on time like clockwork but since receiving this vaccine there were menstrual changes. It was nonexistent. Head throbs all the time. This vaccine has completely ruined patient system and had pain where patient middle finger was almost amputated when patient was a child, patient called it the phantom finger pain that day but it went away the next day. Patient normally pump 5 ounces each breast after feedings, but only obtained approximately 1 ounce from both breasts. On an unspecified date in Jul2021, 3rd to 5th days patient controlled Melasma started to move and grow on my face. The fever went away by the middle of day three. Injection site pain went away by the fifth day. Nausea stops on day three. On 12Jul2021, 6th day patient experienced a small rash started on my middle of patient chest above heart but went away the next day. Patient felt slightly dizzy and out of breath. Joint and muscle pain was easing. On 16Jul2021, patient had severe swelling on my right knee to my feet, swelling stomach, and feeling out of breath going up and down the stairs. Melasma grew all over my face. Headache feels horrible. On 22Jul2021 today patient started swelling legs and feeling unwell. On 19Jul2021 patient had gum nerve pain and bleeding gums when flossing. This day patient tasted blood. Patient head hurts. Patient was healthy before this vaccine and what were patient four small children going to do without their mommy. Patient baby will not nurse from patient and depletion of breast milk was worse, body swelling, leg swelling when walking, sinus congestion, eye pain, neck pain, feeling unwell, headache, weakness when opening unopened jars, difficulty breathing, blood pressure goes up and down, skin was red where the Melasma was located, Melasma was growing all over face and moving so painful, and pealing/feeling hot where Melasma was on face. By the way, Patient wasn't having any of these symptoms except occasional sinus congestion. The patient underwent lab tests and procedures which included blood pressure measurement: goes up and down on 22Jul2021, blood test: unknown on 19Jul2021, body temperature: 100 on 08Jul2021. It was reported no and unknown that therapeutic measures were taken for the events experienced. The outcome of fever, Nausea and Pain in fingers was recovered on 09Jul2021, for Injection site pain was recovered on 11Jul2021, for rash was recovered on 13Jul2021, for Muscle pain and joint pain was recovering, for Gum bleeding and Gum pain was unknown and for all rest events was not recovered.

after first shot, massive headache for 3 days, then deep bone aches that set in and were still going as of 7/8/2021. the bone aches were so severe that they impacted walking and movement. I was also extremely lethargic & was taking naps when I never do this. in addition, fingers, feet and ankles swelled significantly. this required treatment of steroids by rheumatologist.

A 63-year-old male patient received the first dose of BNT162b2 (PFIZER-BIONTECH COVID-19 mRNA VACCINE; Lot number: 59267-1000-01), via an unspecified route of administration in the left arm on 15Mar2021 at 12:45 (at the age of 63-years-old), as a single dose for COVID-19 immunisation. Medical history was not reported. Concomitant medication included atorvastatin (MANUFACTURER UNKNOWN) at the dose of 10 mg. The patient was not allergic to medications, food and other products. Prior to vaccination, the patient was not diagnosed with COVID-19. The patient did not receive any other vaccines within four weeks, prior to the vaccination. Since the vaccination, the patient had not tested for COVID-19.On 16Mar2021, the patient experienced sciatic pain. The patient did not receive any treatment for the sciatic pain. The event did not result in doctor or other healthcare professional office/clinic visit, and emergency room/department or urgent care. The clinical outcome of the event of sciatic pain was not recovered at the time of reporting. No follow-up attempts are needed. No further information is expected.

Left arm tingling and numbness and skin mottling. Symptoms lasted appx 5 minutes then began feeling lightheaded and dizzy with headache and mild nausea. Patient moved to recovery area, laid down on stretcher and provided cool air and ice-pack. Nausea improved, able to sip water, monitored for another 30 minutes and then felt normal. Continued to monitor until 3:10 pm when left.

right after the vaccine, I had heart palpitation and a headache but I thought it was my anxious acting up. Later that night, my arm became quite sore but I got itchy and the headache became a severe migraine so I took a Tylenol and the Rizatriptan 4 hrs later. The following day, I could not get out of bed with severe body ache, light headedness, heart palpitation and slight difficulty breathing. I have been in bed since but the heart palpitation and more difficulty breathing and wheezing are intermediate but strong at onset. I am also still itchy.

A 71-year-old male patient received bnt162b2 (BNT162B2), dose 1 via an unspecified route of administration at the age of 71-years-old, administered in Arm Left on 26Feb2021 13:00 (Lot Number: EN6202) as single dose for COVID-19 immunization. The patient medical history was not reported. The patient had no known allergies. Concomitant medication included ibuprofen taken for an unspecified indication, start and stop date were not reported. The patient received 2nd Shingrix vaccination on 24Jan2021. The patient experienced "foot became paralyzed", "a deep gripping pain in the diaphragm area", "leg experienced increasing pain", "increasing pain from foot to hip", "left foot grew completely numb", "breathing labored", "weakness in left leg", "cramping of calf muscle", "chest cavity pain", "ribcage pain", chills, headaches on 26Feb2021 13:45. The patient also experienced "skin hypersensitive on my upper back" and "back sensitivity" on 2021. The patient reported that, "There were no immediate reactions during the 15 minute wait period. Forty-five (45) minutes after my injection I was hit with a rush of side effects. The first was a deep gripping pain in the diaphragm area. While seated, my left leg experienced increasing pain from foot to hip. In short order, my left foot grew completely numb and this extended up my calf to my knee. My foot became paralyzed. The intense leg pain remained steady. Breathing labored. The left foot and calf numbing and paralysis finally subsided 4.5 hours after injection. Saturday through Wednesday, 28Feb to 03Mar2021: Weakness in left leg. Cramping of calf muscle. The chest cavity pain began to intensify. Chills arrive plus daily headaches. During this reaction period: NO fever, nausea, diarrhea, loss of taste / smell, sore throat, congestion or runny nose. Headache, chills, joint & ribcage pain remain. Skin hypersensitive on my upper back. 15Mar2021: Day 17: Chest pains left side, mild headache and some back sensitivity with chills remain." The adverse event started on 26Feb2021 02:30 AM (as reported). Adverse events did not result in either emergency room visit or physician office visit. No treatment received for events. The outcome of the events "foot became paralyzed" and "left foot grew completely numb" was recovered on 26Feb2021 17:30 (left foot and calf numbing and paralysis finally subsided 4.5 hours after injection) while for other events was not recovered. The patient was not diagnosed with COVID prior vaccination. The patient was tested for COVID post vaccination (LUMIRADXSARS-COV-2-AG TEST) via nasal swab on 06Mar2021 with negative result. The event "foot became paralyzed" was considered as serious (medically significant).

A 57-year-old female consumer received first dose of BNT162B2 (PFIZER-BIONTECH COVID-19 VACCINE) on 11Jan2021 at 04:30PM in left arm at single dose for COVID-19 immunisation at the age of 57-year-old. Lot number was 59267-1000-01. Medical history included no gall bladder, ongoing bloating and uncomfortable like you over eat and have tight pants in on 11Jan2021 at 02:00PM. Patient was not pregnant. Past drug history included allergy to codeine. Concomitant medications included venlafaxine, verapamil, tocopherol (VITAMIN E), ascorbic acid, biotin, folic acid, iodine, pantothenic acid, pyridoxine hydrochloride, retinol, vitamin b12 nos, vitamin d nos, vitamin e nos, zinc (CENTRUM), atenolol (ALTENOL), zopidem tartrate (ZOLPIDEM). On 11Jan2021 at 02:00PM (also reported: wishing 2 minutes, as reported), inside stomachs and gut, the patient felt it spread over that area (not warm, not painful, weird) and afterwards that night bloating and uncomfortable like you over eat and have tight pants in, then the next day (on 12Jan2021, also reported on 11Jan2021 at 02:00PM, as reported) she had a gastro-intestinal (GI) reaction with spasms that were very painful (something that happens very seldom and the patient was sitting at her desk working so no irritable factors) so she had to take strong meds because this GI spasm was very painful and hurt when you breath, so she took strong medication (hyosymin and mepitab). The patient recovered with lasting effects from abdominal distension, the patient recovered from other events.

A 13-year-old male patient received bnt162b2 (PFIZER-BIONTECH COVID-19 VACCINE, solution for injection), dose 1 via an unspecified route of administration, administered in arm left on 14Aug2021 at 16:00 (Batch/Lot Number: 59267-1000-01) (age at vaccination 13-year-old) as dose 1, single for covid-19 immunisation. The patient medical history and concomitant medications were not reported. The patient previously took omnicef [cefdinir] and experienced drug hypersensitivity. The patient did not receive any other vaccines within four weeks prior to the vaccination. The patient did not received any other medications within 2 weeks of vaccination. Prior to the vaccination, the patient was not diagnosed with COVID-19. Since the vaccination, the patient had not been tested for COVID-19. The patient experienced uncontrollably vomiting, nausea, diarrhea, headache and chills on 15Aug2021 at 18:00. The adverse events resulted in emergency room/department or urgent care for fluids, EKG and Zofran. Therapeutic measures were taken for all the events. Medical history was not reported. The patient had no known allergies. Prior to the vaccination, the patient was not diagnosed with COVID-19. The patient did not receive any other vaccines within four weeks prior to the vaccination. Concomitant medications included fexofenadine hydrochloride (ALLEGRA), vitamins (MANUFACTURER UNKNOWN) and herbs (MANUFACTURER UNKNOWN) for an unknown indication from an unknown date and unknown if ongoing. The patient previously did not take any drug in the past. On 03Apr2021, the patient had very bad insomnia, anxiety, extreme fatigue and brain fog for over a week after the vaccine. Later patient had night sweats. The events resulted in doctor or other healthcare professional office/clinic visit. Therapeutic measures were taken as a result of anxiety and included treatment with lorazepam (MANUFACTURER UNKNOWN) and homeopathic remedies. Since the vaccination, the patient had not been tested for COVID-19. The clinical outcome of the events very bad insomnia, anxiety, extreme fatigue and brain fog was recovered on an unknown date in Apr2021. Muscle spasms, a sudden, brief, unintended (involuntary), and usually painful contraction of a muscle or group of muscles, specificaly in left and right legs and feet. In addition to torso or core area. Spasm occurred every few minutes, lastinng 30 seconds to 90 seconds for 15-18 hours. Caused extreme pain and inability to do any daily activities and inability to sleep. After approximately 15 hours, Interval of spasms increased gradually until reaching baseline in approximately 36-42 hours. Currently taking muscles relaxant (Baclofen and Tizanidine) for intermittent spasms related to Multiple Sclerosis.

A 61-year-old non-pregnant female patient received the first dose of BNT162b2 (PFIZER-BIONTECH COVID-19 mRNA VACCINE; Lot Number: 59267-1000-01) via an unspecified route of administration in the left arm on 01Sep2021 at 16:00 (at the age of 61-year-old) as a single dose for COVID-19 immunisation. Medical history included high blood pressure (treated). The patient had no known allergies. Prior to the vaccination, the patient was not diagnosed with COVID-19. The patient did not receive any other vaccines within four weeks prior to the vaccination. Less than 1 min after vaccine, pt was full body sweats, feeling lightheaded, disoriented and near fainting. raised legs, applied cold compress x 2, bp stable gave cool water, advised to remain in booth - pt/spouse denied calls for emergency assistance due to anxiety - 10 mins feeling much better, slightly pale gave snack and powerade drink - bp stable. 30 mins came out of booth and stood for several minutes while I gathered more info and assessed ability to stand. they left to go home. follow up call 8 hours post reaction - patient rested, ate meal (had been fasting this a.m.) hydrated, feels fine

I had first experienced mild Covid symptoms July 15, 2021 that evolved into moderate Covid two days later. I was bed ridden with Covid for seven days. Ten days after onset of Covid symptoms (Dry cough, chills, headach, loss of smell) I took a PCR Test on July 25. The results came back positive for Covid-19. On August 10, seventeen days post Covid test, I took first dose of Pfizer vaccine. Eleven hours latter I was immediately hit with a sudden onset of flu-like symptoms (Chills, body ache, headace) which sent me to bed through the entire next day, Now two days latter I am out of bed but very weak and light headed.

A 25-year-old non-pregnant female received second dose of BNT162b2 (PFIZER-BIONTECH COVID-19 mRNA VACCINE; lot Number/batch number: 59267100001, expiration date: unknown), via intramuscular route of administration in the left arm on 27Jan2021 at 09:00 (at the age of 25-year-old) as 2nd dose, single dose for COVID-19 immunization. Medical history included asthma. Patient had no known allergies. Prior to the vaccination, the patient was not diagnosed with COVID-19. Since the vaccination, the patient had not been tested for COVID-19. Concomitant medications included escitalopram, IUD. The patient previously received first dose of BNT162b2 (PFIZER-BIONTECH COVID-19 mRNA VACCINE; lot Number/batch number: 59267100001, expiration date: unknown), via intramuscular route of administration in the left arm on 06Jan2021 at 02:00 as a single dose for COVID-19 immunization. The patient did not receive any other vaccines within four weeks prior to the vaccination. On an unspecified date in Jan2021, about 18 hours after the 2nd dose (1 AM), the patient experienced fever, severe chills, diarrhea, nausea, headache, body aches, and I was prevented from sleeping then it lasted about four hours before she got back to sleep. She woke up with a headache, stiff neck and generally felt "hung over" the next day. Advil was relieving at this point. The outcome of all the events was recovered. Follow-up attempts are completed. No further information is expected.

Intense pain in right shoulder capsule/deltoid for 3 days. I could not raise my arm it was so painful. Two days after vaccination had swelling in both wrists and both hands and fingers. Seeming arthritic condition. Never had any type of arthritis or swelling of hands or wrists. Slight pain in hands and inability to grip or hold items. First took acemetacin for several days with no relief. then tried normal aspirin for several days. After two weeks someone suggested taking ibuprofen. I took 200mg per day and found slight relief. Contacted my PCP on 2/28/2021 to request a video visit that occurred on 3/3/2021 at 8:00 am. Doctor agreed that swelling and arthritic condition was the result of vaccination and advised to take 600 mg of ibuprofen every 8 hours for a week. Have had some relief from this medication. Doctor advised not to take the second vaccination shot.

Relevant medical history included: "known allergies: Penicillin" (unspecified if ongoing). The patient took concomitant medications. The following information was reported: HORMONE LEVEL ABNORMAL (non-serious) with onset 19May2021 08:30, outcome "recovering", described as "Adrenaline feeling through whole body lasting approx. 2 weeks."; INSOMNIA (non-serious) with onset 19May2021 08:30, outcome "recovering", described as "Had trouble sleeping"; MENSTRUAL DISORDER (non-serious) with onset 19May2021 08:30, outcome "recovering", described as "Menstrual period lighter than normal and lasted 15 days". A 43-year-old non-pregnant female patient received BNT162b2 (PFIZER-BIONTECH COVID-19 mRNA VACCINE; Lot Number: 59267-1000-01) via an unspecified route of administration in the left arm on 20Apr2021 at 12:00 (at the age of 43-years-old) as a single dose for COVID-19 immunisation. Medical history included penicillin allergy. Prior to the vaccination, the patient was not diagnosed with COVID-19. The patient did not receive any other vaccines within four weeks prior to the vaccination. Concomitant medications included propranolol (MANUFACTURER UNKNOWN), taken for an unknown indication from an unknown date and unknown if ongoing. On 22Apr2021 at 00:00, 2 days after the shot, the patient started menstruating 14 days earlier. The event did not result in doctor or other healthcare professional office/clinic visit, and emergency room/department or urgent care. Therapeutic measures were not taken as a result of the event. ADMINISTRATION ERROR! I went to the pharmacy to get my flu shot--- and at 3:10 p.m. on September 16th 2021 was given INSTEAD --- BY ACCIDENT OF THE TECHNICIAN---- THE PFIZER VACCINE INSTEAD OF THE FLU SHOT. I had the PREVIOUS 2 Pfizer VACCINES in FEBRUARY 2021 and a 3RD PFIZER (recent booster) in AUGUST 2021... ( I had my Pfizer vaccines in February 2021 and I had my booster August 8th 2021. ) SO THEREFORE I HAVE NOW RECEIVED A 4TH(FOURTH) PFIZER VACCINE BY ACCIDENT on September 16th 2021. That is 4 VACCINATION DOSES in all of the PFIZER VACCINES . I'm concerned because of getting that extra vaccination. I also got the flu shot afterwards on 9/16/2021-- because I asked that I receive the flu shot-- since that is what I came to the pharmacy to get originally before the mistake was made.

A 59-years-old non-pregnant female patient received BNT162B2 (PFIZER-BIONTECH COVID-19 VACCINE, Solution for injection. Batch/Lot Number: 59267-1000-01), dose 1 via an unspecified route of administration, administered in left arm on 21Apr2021 at 14:45 (at the age of 59-years-old) as DOSE 1, SINGLE for COVID-19 immunization. The patient medical history was not reported. Concomitant medications included valaciclovir hydrochloride (on hand for whenever there was a recurrence, so patient was taking it) (VALACYCLOVIR HCL) and loratadine (for seasonal allergies) (ALAVERT) taken for seasonal allergy, start and stop date were not reported received in two weeks. The patient previously took sulfa [sulfanilamide] (Known allergies: Rash with sulfa) and experienced rash, shingrix for immunization (did have the full course of the Shingrix vaccine in the past year). The patient was not pregnant at the time of vaccination. No COVID prior vaccination. Facility type vaccine at pharmacy or drug store. On 25Apr2021 at 07:00, the patient experienced reactivation of thoracic shingles-just mild so far. Adverse event treatment include Valacyclovir. No COVID tested post vaccination. The outcome of the event was unknown.

Medical history included attention deficit hyperactivity disorder (ADHD), autism spectrum disorder, asperger's syndrome and known allergies: Cashews. The patient did not receive any other vaccines within 4 weeks prior to the COVID vaccine and received other medications ritalin, elavil, aspirin and multivitamin. Patient had not been diagnosed with COVID-19 prior to vaccination and had not been tested since the vaccination. Lab tests and procedures which included blood pressure measurement: same pattern appeared again about 6 weeks later, along with pulse and blood pressure irregularities and heart rate: same pattern appeared again about 6 weeks later, along with pulse and blood pressure irregularities both on unspecified date in 2021. On unspecified date in 2021, the patient experienced sudden onset of very unpleasant ice-cold feet and knees, which could not be remedied with blankets and lasted all night and reoccurred only at bedtime for days, same pattern appeared again about 6 weeks later, along with pulse and blood pressure irregularities and thyroglossal duct cyst. Within 12 hours after the first dose: Upon retiring to bed, patient experienced the sudden onset of very unpleasant ice-cold feet and knees, which could not be remedied with blankets and lasted all night and reoccurred only at bedtime for days. The same pattern appeared again about 6 weeks later, along with pulse and blood pressure irregularities. About 2 months after the last vaccine dose, a first occurrence thyroglossal duct cyst (a 'birth defect' which appears in early life, almost never in the elderly) arose on the left side between the hyoid bone and the thyroid cartilage. Adverse events resulted in doctor or other healthcare professional office/clinic visit, disability or permanent damage and Congenital anomaly or birth defect. No treatment was received for adverse events. At the time of this report, outcome of the events was not recovered, except thyroglossal duct cyst was unknown .; Sender's Comments: Based on current information available, the event cold feet, pulse irregular, blood pressure abnormal were considered related and thyroglossal duct cyst represented intercurrent condition in this patient with advanced age and with congenital anamoly. unrelated to Bnt162b2. More information is needed to better assess the case. The impact of this report on the benefit/risk profile of the Pfizer product is evaluated as part of Pfizer procedures for safety evaluation, including the review and analysis of aggregate data for adverse events. Any safety concern identified as part of this review, as well as any appropriate action in response, will be promptly notified to RAs, Ethics Committees, and Investigators, as appropriate

20mins after taking the shot, I could feel my throat start to swell up. I could still breathe, but I could barely swallow water.. I went to an Urgent Care. They provided me diphenhydramine (oral - I refused a shot since i had just taken a shot that went bad). After 30mins I could finally drink some water. At this point, I started to have numbness/loss of feeling in my left arm (wrist to shoulder), left side of my neck and face, the left side of my torso, and part way down m left leg. Nothing was done by Urgent Care (they claimed this was simply anxiety). My blood pressure was then taken and was 152/100. That is not normal for me (I do have some spats with pre-hypertension, but nothing beyond 135/80-85). The urgent care once again said this was due to anxiety. Since I could breathe I left. I then contacted my Primary Care. I told them I would wait until they had an appointment. That if anything go worse, I would go to the ER. I have a BP device so I could keep tabs on my BP .I saw, FNP (Cary Healthcare Associates) on 9/7/2021. She found that my blood pressure had finally gone down into a good range. She couldn't see anything really closed up in the upper half of my throat. She's not able to explain the numbness beyond the reports of people stating they've had brief paralysis of the face after the Pfizer/Moderna shot.

This consumer(patient) reported for himself thata 41-years-old male patient received first dose of BNT162B2 (Formulation: Solution for injection, Batch/Lot number: 59267-1000-01), via an unspecified route of administration, administered in Arm Right on 25Jun2021 18:00 (Age at vaccination 41 years) as single dose for COVID-19 immunisation. Medical history included hiatus hernia and diverticulitis. Concomitant medication(s) included paracetamol (TYLENOL). Patient had no known allergies. On 25Jun2021 18:00, the patient experienced atrial fibrillation was detected but not sure yet if it was an underlying condition or caused by vaccine; tunnel vision; extreme sense of coldness and face planted to the floor. Patient was received treatment for events. It was reported that after patient received shot and exited the medical room, he had extreme sense of coldness come over me, tunnel vision, then face planted to the floor. Ambulance was called, patient regained consciousness about a minute or so later, and my chin was split open. They took me to ER, stapled my chin, and ran tests. Patient did not receive any other vaccines within 4 weeks prior to the COVID vaccine. Prior to vaccination, patient was not diagnosed with COVID-19. Since the vaccination, patient has been not tested for COVID-19. Other medications the patient received within 2 weeks of vaccination include Tylenol. Patient received Cardizem 1/day till doc follow-up visit. The clinical outcome of all events were unknown. Information on the lot/batch number has been requested.

5 days following my first Covid vaccine I developed pronounced olecronon bursitis for the first time. The swelling was not resolved for weeks - - but once I took a continuous course of anti-inflammatory for over a week it did resolve. My doctor suggested that I should include this experience here as a potential consequence of the vaccine. The elbow does look changed, following the inflammation process - - there appears to be some remodelling/shape change.

I get a bad taste in my mouth when i eat certain foods. It started the first time i ate just after the shot. The taste is not in my mouth all the time, just when i eat certain foods. Usually it is red meat that causes the taste. The taste is hard to describe. It is bad but does not make me sick to my stomach. It has been two weeks since the shot and the symptom os still the same.

Red triangular raised bump patch (non-itch) of skin from right ear to the middle of the throat. Appeared by late afternoon of the day the vaccine was administered. The rash lasted 4 to 5 days, turn brown and skin sloughed off, skin over the area of the throat still feels different now. Also I have been experiencing a change in smell sense. I smell a wood/ash smell numerous time through out the day since receiving the vaccination, I can kind of take it when I smell the ash/wood smell.

Unidentifiable taste in mouth w/in 5 minutes, burning sensation bottoms of both feet w/in ten minutes, lips became numb on way home, (reside 5 minutes away) burning sensation continued to below knees within hour, dissipated during day but not completely. Similar to adverse reaction years ago to Avelox.
