Dizziness Feeling abnormal Nausea Pallor Syncope

Symptomtext

This information was reported to this writer. Immediately after receiving the second dose of the Pfizer vaccine, the patient reported a "weird sensation" in his chest. The patient became pale, and reported nausea and lightheadedness. According to the vaccinator it seemed as though the patient went into a syncopal episode. A nurse attempted to do a sternal rub to wake the patient up but was unsuccessful. The patient was then laid onto the ground and woke up; attempted to sit patient up and then went into another syncopal like episode. EMTs were notified and were on their way to attend to the patient. Patient's feet were elevated. No history of vaccine reaction. Patient had eaten and had fluids 2 hours prior to the vaccine. Patient was sat up again with assistance but immediately felt lightheaded and had another syncopal like episode. He was again placed in a supine position with feet elevated. Immediately reported improvement of symptoms. Patient had several sips of juice and was placed on a stretcher and taken to a back room. Patient had mom and sister at his side. As the patient remained in supine position, he denied nausea and lightheadedness; no chest pain, no dizziness. Patient was alert and oriented. Remained pale in color according to his father who just arrived. Per the advice of the EMTs, patient was transferred to hospital per ambulance.

A 52-years-old female patient received BNT162B2 (Formulation: Solution for injection; Batch/Lot Number: EW0179-H), dose 1 via an unspecified route of administration, administered in Arm Left on 05Jun2021 11:45 as DOSE 1, SINGLE for covid-19 immunisation. Patient medical history included Migraines 1-2x/month from an unknown date and unknown if ongoing. The concomitant medication was not reported. Not known allergy. The patient did not receive any other vaccines within 4 weeks prior to the COVID vaccine. On 05Jun2021 12:15 PM patient experienced 25 minutes after injection, left side of face was numb, loud tinnitus, severe pain in left neck, jaw, and shoulder; few days later, numb tingling in face and tongue, in both legs below knees, both feet, both arms below elbow and both hands; bouts of brain fog and muscle aches; all described symptoms have persisted for 7 weeks without stopping. The patient underwent lab tests and procedures which included laboratory test: unknown results, magnetic resonance imaging: unknown results on an unspecified date. Ae resulted in doctor or other healthcare professional office/clinic visit. The patient received treatment with Antiviral medication. Prior to vaccination, the patient was not diagnosed with COVID-19. Since the vaccination, the patient had not been tested for COVID-19. The outcome of the event was not recovered.

Symptomtext

Pt. arrived for first Pfizer vaccine, pt. alerted staff to anaphylaxis risk to bees and trees but stated she has never had an adverse reaction to vaccinations. Pt. has not used her epi pen in 10 years. Approx 10 minutes into observation pt. complains of lip tingling, swollen lips and states "half of face is swollen". EMS contacted and vital signs stable except for elevated blood pressure. Pt. given 25mg Benadryl and water. Second dose of Benadryl 25mg given at 1733. Pt. educated to seek medical attention if reaction increases. Cleared by EMS, pt. exited facility in stable condition with partner.

My 17 year old daughter had her second Pfizer shot on Sunday, May 16 at 10AM. On Wednesday, May 19the she woke up and said that she had a terrible time in the middle of the night with a tight chest and she couldn't catch her breath. She wouldn't go to Urgent Care with me so I told her that we had to call the 24 hour nurse line (on the back of the insurance card). I called and she spoke with a nurse, then spoke with a doctor on the phone who told her to go the doctor if her symptoms got any worse.

Symptomtext

Pt. with parent for second pfizer vaccine. Pt. had no reaction with first vaccine. After 20 minutes, pt noted hives on his legs and chest. EMS alerted. No SOB, no difficulty breathing. No hx of anaphylaxis from vaccines. Pt. given 25mg benadryl at 1657. After rest of allotted wait time pt. states hives are decreasing and pt. has no worsening symptoms. Pt. exited facility with parent.

Symptomtext

Client presented for 2nd dose of Moderna today but was inadvertently administered a dose of Pfizer vaccine. Client was notified of the error and was given proper patient education about the administration error. Client was notified that they will be followed up with by a member of our staff in 2-3 days to ensure that they are doing alright. Error documented in internal reporting systems.

Pt reported "I feel like my heart is beating too fast" RN took radial pulse 94, EMS was called to evaluate. VS were WNL, Pt denied feeling like she was having an allergic reaction and symptoms improved with sips of H20 provided by clinical staff. Pt able to leave after completing 30 min observation.

Information given to this writer as follows: 1 dose of Pfizer vaccine wasted. Pt was here to receive 2nd dose of Pfizer vaccine. Vaccinator administered vaccine into L deltoid, needle into arm, vaccine injected, and vaccinator retracted needle. When needle retracted, most of the vaccine leaked out of arm. Vaccinator states Pt received less than 50% of vaccine, and so with supervisor instruction, vaccinator revaccinated Pt with new full dose into R deltoid.
