Symptomtext

Taken by ambulance from home to hospital. Husband reports symptoms to be slurred speech, overall weakness in body, thrashing around in bed. Approx. 6am on 11-18-22. Admitted to hospital for 1 day and did testing. Diagnosis was Ischemic Stroke. Follow up with Primary physician and other specialists.

Symptomtext

Client was present in the clinic for her booster dose of Pfizer (LOT FK9894-A). There was an attempt to deliver the dose to the client?s left arm, but the needle became disconnected form the syringe during the vaccination and the client received less than half a dose. The vaccinator, LPN, reported the occurrence to the clinical site supervisor RN PHSS and the pharmacist (the clinician who had originally prepared the dose). The client and her guardian were made aware and the opposite arm was vaccinated at 1706 per protocol.
