Symptomtext
Patient seen in retail health clinic 6 days after vaccine. Mother states has seen PCP twice for vaccine reaction and coming to clinic for strep test to rule out strep throat. Reports after receiving vaccine patient had fever, chills, fatigue, ear pain, rash, heart palpitations, abdominal pain, and mucus in stools. Has been giving acetaminophen for fever and benadryl for itching as directed by PCP. Afebrile at time of visit, fine papular rash observed, slight erythema to throat. Strep test negative at visit.
