Symptomtext

The patient passed out after walking through the store toward the bakery to get something to eat with his parents and siblings. The patient hit is head on the tile and stated his head heard afterwards. The patient was awake when the pharmacist arrived at the scene and denied dizziness, nausea or any other symptoms. The vaccine administration site was observed and not swollen, red or hot. The patient was given an ice pack for his head and a medic at the scene checked his vitals and reported his heart rate, oxygen and blood pressure were within normal limits. The patient had eaten breakfast that morning but had not eaten lunch yet. The patient ate some crackers and drank some water and was able to leave the store on his own two feet.

Symptomtext

About 5.5 hours after injection I had a sudden onset of symptoms starting with cramps and nausea. I nearly fainted at work while seated at my desk. Symptoms were dizziness, faint, blurred vision, nausea, stomach cramps, chills, clamminess, and fever. The symptoms improved slightly with Tylenol for about 4-5 hours and then became severe again. Throughout the night I had chills/shaking and sweats and body aches. The next day I had a low fever that was better with Tylenol and severe muscle pain in my lower back and around my spine. The third day (today) I had severe neck and shoulder pain.

Symptomtext

Patient order was to receive Moderna Bivalent dose Covid vaccine on 12/29/2022. The medical assistant administered 0.25 ml of the Moderna monovalent vaccine in error. The error was discovered on 12/30/2022 by a nurse checking documentation. The ordering physician and the patient were notified. The patient reported no adverse reaction other than the mild expected side effects of the Moderna vaccine. The patient was advised to return after 2 months to receive the Bivalent vaccine as recommended by the CDC in this instance. The error was documented, the Moderna monovalent vaccine was labeled with a bright orange label and moved to a different location in the freezer. Staff re-education was held with the administering.. ...MA, and all other MAs and nurses. A CDC "At A Glance" poster about the schedule and dosing of the Moderna Covid vaccines was placed on the wall in the vaccine room.
