Respiratory distress. He is 60% on nonrebreather, tachycardic in the 140s, normotensive. He is placed immediately on BiPAP 10/5 and does have improvement of his oxygen saturation. He continues to be tachycardic and is given Cardizem 10 mg IV for concern of A. fib with RVR, continued on Cardizem drip, heparin and broad-spectrum antibiotics.
