10:28 AM, APPROXIMATELY 1 MINUTE AFTER LAST VACCINE (GARDASIL), PATIENT LOSS CONSCIOUSNESS. EMERGENCY PROTOCOL INITIATED. PATIENT AWAKENED APPROX. 15 SECONDS LATER. WAS CONTINUOUSLY MONITORED BY NURSES ON SITE. 1029--BP INITIALLY WAS 137/92, HR 88. LAST BP AT 10:39 WAS 115/84, HR 77. PARENT REPORTS WILL CONTINUE TO MONITOR AT HOME FOR ANY ADVERSE EFFECTS. PATIENT AMBULATED WITHOUT ASSISTANCE WITH PARENT OUT OF CLINIC.

Syncope

Symptomtext

At 11:19 am Client was extremely anxious prior to vaccination. After vaccination client had a syncope episode in the chair, head lowered feet elevated. VS taken, after a few seconds client was responsive juice and food provided. Client did not eat or drink for the day. VS monitored until completely oriented. escorted to waiting area with parents. Left at approximately 1pm in good health.

Chills Dizziness Headache Nausea Tremor Vomiting projectile

Symptomtext

Patient presented to the office for a possible vaccine reaction. She had the meningococcal B and flu vaccines this morning. The vaccines were given around 10:30am this morning. About an hour ago (around 1:00pm), she started to have chills. Shortly afterward, she started shaking uncontrollably. Her mom was starting to get worried, so brought her back to the clinic. She is now feeling lightheaded/dizzy, nauseated, and getting a headache. Denies erythema, itching, or rash. Denies itching, tingling or edema of lips, tongue, or throat. Denies hoarseness, shortness of breath (dyspnea), chest tightness, cough, or wheezing. Denies abdominal pain or diarrhea. Denies syncope, altered mental status, chest pain, or palpitations. Denies lethargy, or confusion. While checking vitals, she began projectile vomiting. HR 120bpm BP 110/70 Spo2 97% T 98.8 EMS called. Epinephrine 0.3mg IM given. Sent to ER via ambulance.

Symptomtext

Pain upon touch all down my right leg, on the inner/top of my thigh and down the right/front side of my shin from my knee through my ankle. It is not muscular, only hurts when touched. Numbness and tingling in the leg at times as well. They did an ultrasound and I was told that it was not a DVT but that it could possibly be superficial Thrombophlebitis. This started 2 days after the shot and is still paining me a week later.

Symptomtext

Patient received HPV and influenza vaccine on a mobile medical van. After vaccine administration client reported feeling lightheaded and had delayed verbal responses. Client observed to sway in his seated position but did not fall or lose consciousness. Blood pressure, heart and oxygen saturation vitals were gathered. Patient given snacks and bottled water. Cooled with ice packs and elevated legs. Client still reported feeling lightheaded. Patient walked into school accompanied but with no assistance. School Principal was notified of condition and student's family was notified. In the school patient given milk and cereal but still was swaying with delayed verbal responses and reported feeling dizzy. Vitals were taken again. Decision to call 911 for further assessment was made. Relayed patient's condition to paramedics on ambulance. Family member accompanied student on ambulance to hospital. Patient never appeared to lose consciousness, remained verbally responsive and answered questions appropriately throughout whole encounter.

Patient was inadvertently administered the 10mcg Pfizer Covid 19 vaccine instead of the 30mcg COMIRNATY that is recommended for his age. No adverse events post vaccination reported. Parent is aware of error and Pfizer Medical consulted for advice. Advised that administration of age appropriate vaccine (30mcg COMIRNATY) is recommended per CDC and Pfizer guidelines.

APN advised patient desired vaccines, indicating which vaccines to be given by circling names on vaccine sheet. It was noted patient had Tdap in 2012 and needed TD rather than Tdap. When vaccines were pulled from pharmacy, Tdap and TD both were pulled and given. Patient notified 09/15/2023 of error and denies any adverse signs or symptoms.
