patient got a dose after temp excursion; This non-serious case was reported by a nurse via call center representative and described the occurrence of incorrect storage of drug in a 61-year-old male patient who received HBV (Engerix B) (batch number 29RN7, expiry date 25-FEB-2024) for prophylaxis. On 24-JAN-2023, the patient received Engerix B. On an unknown date, an unknown time after receiving Engerix B, the patient experienced incorrect storage of drug (Verbatim: patient got a dose after temp excursion). The outcome of the incorrect storage of drug was unknown. This report is made by GSK without prejudice and does not imply any admission or liability for the incident or its consequences. Additional Information: GSK receipt date: 9-Mar-2023 Adverse event contains all reported information. The patient received a dose of Engerix-B after a temperature excursion on negative 0.1 Celsius, which led to incorrect storage of drug.

Engerix-B 20 mcg administered instead of 10mcg dose; Engerix-B 20 mcg administered to 15y/o; This case was reported by a pharmacist via call center representative and described the occurrence of overdose in a 15-year-old male patient who received HBV (Engerix B adult) (batch number 29RN7, expiry date 25th February 2024) for prophylaxis. Co-suspect products included hepatitis B vaccine pre-filled syringe device (Engerix B Pre-Filled Syringe Device) injection syringe for prophylaxis. On 12th October 2022, the patient received Engerix B adult 20 ug and Engerix B Pre-Filled Syringe Device. On 12th October 2022, unknown after receiving Engerix B adult and Engerix B Pre-Filled Syringe Device, the patient experienced overdose and adult product administered to child. On an unknown date, the outcome of the overdose and adult product administered to child were unknown. This report is made by GSK without prejudice and does not imply any admission or liability for the incident or its consequences. Additional Information: GSK Receipt Date: 28-OCT-2022 Reporters comment: The reporter stated that on 12th October 2022, an Engerix B 20 mcg vaccine was administered to a 15 years old patient. The reporter reported that the Engerix B 20 mcg dose was administered instead of a 10 mcg dose. No additional information was reported. The reporter consented to follow up with HCP. Additional supportive information: The patient received 20 mcg vaccine instead of 10 mcg, which led to overdose and adult product administered to child.
