Symptomtext

Morning after vaccine (2/12/2022) patient woke up with severe myalgias and a high fever of 101.8 degrees F. Her arm was red, swollen, and sore at injection site. The next day on 2/13/2022 patient's arm became hard, red, warm to touch down towards her bicep. She still had a fever of 99.5 degrees F. Over the next couple of days her arm continued to be hard, red, warm to touch and also developed fluid under the skin. On 2/16/2022 the patient also experienced hypertension (150's/90's) which is very unusual for her. She went to the urgent care where they confirmed her HTN. Patient reached out to us on 2/17/22 and was feeling better but arm was still sore/red/swollen.

The day after receiving my 2nd shot of Shingrix i had a slight fever, general malaise, an soreness at injection site. Eleven (11) days after my injection i began to have severe itching at my right ankle, this proceeded to my right thigh, under my right arm ,lower back ,onto the top of my buttocks. a rash developed shortly afterwards, the rash was raised and resembled what shingles would look like. There is no weeping from the rash. here is however severe bruise at the top of the crack of my buttocks. My inner right thigh has also developed a rash as well as the area on my left rib cage.. Different patches become severly itchy at different times. not painful but itchy. looks terrible. Looks if I have the shingles.

Symptomtext

Chills, fever, nausea, headache Chills started approx 7pm. Fever with nausea and headache started approx 2am followed by chills then fever then chills and fever again. I took tylenol 2am Symptoms were subsiding by approx 8am. I took more tylenol at 8am and finally got out of bed around noon.

Patient felt dizzy immediately after the shot. Had an episode of non responsiveness but still breathing. Called EMS. Had two more non responsive episodes once when paramedics were here. Was reposonsive but taken to hospital for precaution.

Inappropriate schedule of product administration

Symptomtext

1st dose 11feb2022, 2nd 14mar2022; This case was reported by a pharmacist via call center representative and described the occurrence of drug dose administration interval too short in a 71-year-old male patient who received Herpes zoster (Shingrix) (batch number 2NT5L, expiry date 12th May 2023) for prophylaxis. Concomitant products included RECOMBINANT VARICELLA ZOSTER VIRUS SURFACE GLYCOPROTEIN E (SHINGRIX). On 14th March 2022, the patient received the 2nd dose of Shingrix. On 14th March 2022, unknown after receiving Shingrix, the patient experienced drug dose administration interval too short. On an unknown date, the outcome of the drug dose administration interval too short was unknown. Additional Information: GSK Receipt Date: 12-AUG-2022 Reporter's comment: The registered pharmacist reported that the patient received second dose of Shingrix 31 days later the first dose of Shingrix. The pharmacist inquired should they revaccinate the patient. The doses not two months apart. Registered pharmacist confirmed the second dose was administered at their facility but not the first dose. The reporter did not consent to follow up. Additional Supportive Information: The patient received second dose of Shingrix, earlier than the recommended interval, which led to shortening of vaccination schedule.

reconstituted with unapproved diluent, administered; reconstituted with unapproved diluent, administered; This case was reported by a nurse via call center representative and described the occurrence of wrong solution used in drug reconstitution in a 63-year-old male patient who received Herpes zoster (Shingrix) (batch number 2NT5L, expiry date 12th May 2023) for prophylaxis. On 4th March 2022, the patient received Shingrix. On 4th March 2022, unknown after receiving Shingrix, the patient experienced wrong solution used in drug reconstitution and inappropriate dose of vaccine administered. On an unknown date, the outcome of the wrong solution used in drug reconstitution and inappropriate dose of vaccine administered were unknown. Additional details were provided as follows: The reporter stated that they reconstituted the Shingrix Lyophilized powder with an unapproved diluent then inadvertently administered to the patient in the right deltoid, which led to wrong solution used in drug reconstitution and inappropriate dose of vaccine administered. The reporter wanted to know, how long did the patient had to wait to get the correct dosage. The reporter did not consent to follow-up.
