Symptomtext

(February 2013 received Varivax series). Several days after Shingrix vaccination arm swelled and was painful. A large raised red rash, warm to the touch, appeared, with a tingling itchy sensation. As time is going by tingling sensation is lessing, rash is fading and less raised, is still warm to the touch.

Symptomtext

Patient experienced what he described as shooting pains throughout his body, namely his joints and in his abdomen one month after getting the Shingrix vaccine. The pain is intermittent but has been going on for days now. He reports no other symptoms. He has not seen a physician about this problem yet.

Symptomtext

Patient developed 2 small bruises, one at each injection site about a pea sized. Also had fever for 2 days, max 101.9. Came in today with 2 red marks on arm and claimed they itched 1 was the size of an orange and 1 about quarter sized.

Symptomtext

PATIENT'S WIFE CALLED TO PHARMACY AND SAID HE WOKE UP FROM NAP AND HAVING EXTREM PAIN IN HIS ARM AND SHOULDER OF SHINGLES INJECTION ARM. HE TOOK IBUPROFEN 600MG ABOUT AN HOUR AGO AND PAIN SEEMS NOT GETTING BETTER. ADIVSED PT TO WAIT MAYBE ANOTHER AN HOUR UNTILL IBUPROFEN WORKS AND IF THE PAIN GETS WORSE OR NOT GETTING BETTER, CALL US BACK OR WALK IN TO URGENT CARE TO HAVE DR TO LOOK AT THE INJECTION AREA. PT HASN'T CALLED US BACK YET.

nurse gave a patient just the diluent; nurse gave a patient just the diluent; uses another diluent to reconstitute and administer it; This non-serious case was reported by a nurse via call center representative and described the occurrence of inappropriate preparation of medication in a male patient who received Herpes zoster (Shingrix) (batch number 2Z9N3, expiry date 26-SEP-2025) for prophylaxis. On 11-OCT-2023, the patient received Shingrix. On 11-OCT-2023, an unknown time after receiving Shingrix, the patient experienced inappropriate preparation of medication (Verbatim: nurse gave a patient just the diluent), inappropriate dose of vaccine administered (Verbatim: nurse gave a patient just the diluent) and wrong solution used in drug reconstitution (Verbatim: uses another diluent to reconstitute and administer it). The outcome of the inappropriate preparation of medication, inappropriate dose of vaccine administered and wrong solution used in drug reconstitution were unknown. Additional Information: GSK Receipt Date: 11-OCT-2023 The nurse requested data that can Shingrix be reconstituted with anything other than the diluent it comes with as she was short 1 diluent because another nurse gave a patient just the diluent then uses another diluent to reconstitute and administer it which led to Wrong solution used in drug reconstitution, inappropriate preparation of medication and inappropriate dose of vaccine administered. He got a correct dose as well. Patient received Shingrix adjuvant only followed by a complete Shingrix dose on the same day. The reporter consented to follow up.

Patient experienced numbness of the tongue and a feeling of her tongue feeling "thick". Patient said that the feeling only lasted approximately 60 seconds and resolved on its own without any treatment. There was no swelling noted.
