Symptomtext
Patient picked up medication at her local Pharmacy and hand brought vial to our clinic . Patient was given injection in office . As Medical assistant was logging into patients chart she noticed vial was expired . We did contact the pharmacy Department . Report competed withy VAERS. Meeting with Staff to verify 3 times . Patient was contacted and was discussed with Provider. Patient did come back and received new dose. PER CDC recommendations..
