Within 24 hours, developed fever 101 degrees and body aches that lasted for 3 days. I was unable to get up and move around due to body aches and fever, even while taking my ibuprofen prescription (600 mg) every six hours. I missed 3 days of work. On the 4th day, fever subsided but migraine persisted for 3 more days. This migraine was unresponsive to my migraine medicines (Triptans ) I have never had side effects from any vaccine before (other than a sore arm).

Started out with a painful arm with weakness in affected arm to the point of difficulty in using affected arm. This lasted about a week. Then patient started experiencing a deep throbbing pain in the joints of all the patient's fingers but not affecting her wrists. This event is also present in her knees and ankles. This event is still ongoing. PCP seen advised patient to take OTC Ibuprofen TID PRN.

The day before the injection, I drank a lot of water. The morning of the injection, I continued drinking water. I ate a large breakfast of 2 eggs, wontons, and broccoli. At 10:45 AM, I walked one block to the Pharmacy inside Store. After the injection, I walked around the store. 15 minutes after the injection, I felt dizzy and nearly passed out. The pharmacist brought me a chair and helped me in to it. I asked for water and drank it. Firemen and a paramedic were called and took vital signs. All was normal except blood pressure was 90/60 which indicates low blood pressure. A fireman said I looked flushed. Later readings showed that blood pressure was back to normal. I was not allowed to walk one block home but had to call for a ride. The rest of the day, I felt better but my head hurt. Today, the next day after the injection, I am fine.

She got her vaccine, the day of the vaccine she had just a sore left arm. On Monday 7/24/23 she had what she had was an eye infection. Her eye swollen, had video visit and she diagnosed an eye infection and a sinus infection and put her on antibiotics. On Tuesday she started with nausea, vomiting and diarrhea. On Friday she did not take it anymore, did not want another video visit and her husband took her to the ER. There they officially diagnosed her with shingles. She got an IV, antinausea medicine, they put her on an antiviral pill. Her shingles was in her eyes, she saw an eye doctor on Friday afternoon, again on Monday and she sees him again tomorrow. The eye doctor told her to continue the antiviral and put in artificial tears a few times a day and to stop the antibiotic the ER physician gave her. She has lost 13# in one week. She is also having headaches, which the ER told her was not unusual. The thing that she never knew was that shingles is only one sided. She is also taking Tylenol for the headaches. She is improving with her symptoms, she is not having the nausea, vomiting or diarrhea and able to eat. The spots are drying off and shrinking around her eye. The ones in the eye itself are getting better as she is able to see better, but her eye is still swollen, but is hard to tell. They are also looking for damage to the cornea and that is why she has to see them frequently. She was told to see her PCP if her N/V/D did not stop to come back.

On 14-JUL-2023, the patient received Shingles vaccine. On 14-JUL-2023, less than a day after receiving Shingles vaccine, the patient experienced nausea (Verbatim: Nausea), fever (Verbatim: high fever 101f.), general body pain (Verbatim: Pain throughout body) and inappropriate age at vaccine administration (Verbatim: 48 year's old patient received shingles vaccine). The outcome of the nausea, fever and general body pain were not resolved and the outcome of the inappropriate age at vaccine administration was unknown. It was unknown if the reporter considered the nausea, fever and general body pain to be related to Shingles vaccine. It was unknown if the company considered the nausea, fever and general body pain to be related to Shingles vaccine. Additional Information: GSK Receipt date: 15-JUL-2023 The patient self-reported this case. The symptoms were not treated. On 15 July 2023 the outcome of events was reported as not recovered. Patient came to the pharmacy complaining of sore arm and rash and redness and pain around the injection site following second dose of Shingrix 48 hours ago. She stated she started feeling body ache and dizziness within a couple of hours of receiving the Shingrix and the fever lasted 24 hours despite taking Tyleonol. Her arm was still red around the injection site and painful. She was asked to contact her doctor for further evaluation. She stated she had similar reactions with the first Shingles shot but the adverse reactions are much more severe and lasts longer this time.

PATIENT AND SPOUSE WANTED TO LEAVE AFTER ABOUT 10 MINUTES AND SHE FELT FAINT AFTER TAKING A FEW STEPS. HER SPOUSE HAD HER SIT DOWN IMMEDIATELY. SHE ALTERNATED BETWEEN APPEARING "SPACED OUT" WITH NO LOSS OF CONCIOUSNESS AND AWARENESS EVERY 10 SECONDS OR SO FOR ABOUT 1-2 MINUTES THEN SHE CAME TO AND STATED THAT SHE FELT FAINT. WE APPLIED A COOL PACK TO HER NECK AND SHE DRANK A FEW SIPS OF WATER AND STARTED FEELING MUCH BETTER. AFTER ABOUT 10 MINUTES, SHE SAID SHE FELT FINE. SHE REMAINED SEATED FOR ANOTHER 10 MINUTES OR SO BEFORE LEAVING WITH HER SPOUSE.

No adverse effects, but patient was vaccinated on 9/29/23 and it was noticed on 10/9/23 that a vial of diluent was missing from the Shingrix box, after investigation it was found this patient only received the diluent instead of the diluent mixed with powder vaccine for Shingrix. Nursing giving the shot was unfamiliar with the product and was unaware it needed a powder to be reconstituted. So patient was not actually vaccinated for Shingles correctly. Patient will be contacted to return to clinic to get fully vaccinated for Shingles.

PATIENT (DOB XX/XX/XXXX) REPORTED TO THE PHARMACY TODAY WITH A RED CIRCLE AROUND THE AREA WHERE SHE RECEIVED HER SECOND SHINGRIX VACCINATION. PATIENT WAS INFORMED THAT THIS WAS NOT AN UNUSUAL REACTION, AND THAT APPLYING A COLD PACK WOULD HELP QUITE A BIT. PATIENT WAS ALSO INFORMED TO KEEP AN EYE ON THE REACTION AND IF IT SPREADS, LET US AND HER PRESCRIBER KNOW. PATIENT EXPRESSED UNDERSTANDING AND THANKED ME FOR THE ADVICE OF THE COLD PACK. WILL FOLLOW UP NEXT WEEK.

patient inadvertently received a third dose of Shingrix; This non-serious case was reported by a pharmacist via call center representative and described the occurrence of extra dose administered in a 62-year-old male patient who received Herpes zoster (Shingrix) (batch number 3N743, expiry date 20-AUG-2024) for prophylaxis. Previously administered products included Shingrix (on 30th October 2020 received 1st dose.) and Shingrix (on 31th December 2020 received 2nd dose.). On 13-JUL-2023, the patient received the 3rd dose of Shingrix (left deltoid). On 13-JUL-2023, an unknown time after receiving Shingrix, the patient experienced extra dose administered (Verbatim: patient inadvertently received a third dose of Shingrix). The outcome of the extra dose administered was unknown. Additional Information: GSK Receipt date: 13-JUL-2023 The pharmacist reported that a patient inadvertently received a third dose of Shingrix on 13 July 2023. The reporter states that the patient had 2 doses of vaccine before, did not remember at time of injection and received a 3rd dose which led to extra dose administered. The reporter did not consent to follow up.
