This writer recognized the Lot# of the given Hep B Vx was corresponding to pediatric Vx stock. This was verified with another nurse and brought to the attention of the HD. Pt was immediately contacted about dosage discrepancy and that administration of second 0.5mL dose of Hep B Vx would be necessary to complete a full adult dose (1 ml) of Hep B Vx. Pt arrived at clinic in timely fashion and received the additional Vx and has received full adult coverage for the Hep B (2) Vx. Pt has 3rd dose left to complete full 3 dose course for Hep B. Pt. tolerated administration well.
