Patient called office and stated she had a fever when she woke up on the morning of 2/9/21 she got into the shower and as she was showering she got light headed and got out the shower and she stated she fainted on the bathroom floor.

sweating/dizziness in 10 mins after shot. Extreme chills/fatigue - next 24 hours. Day 1 after shot silver dollar sized bump-red, painful formed near elbow. Whole arm from injection site down swelled up. Radiating pain off and on from armpit down rib cage. By Monday following, doing better.

Received the vaccination. Said immediately felt horrible pain like pharmacist hit a brick wall. There was bleeding which bandaid was applied to. Had to wipe off with tissue. Bruising started middle of night. Swelling has occurred. Pt has iced her arm. Pt has continued to use the arm, She is active with it. Ice helps, but if take ice away pain returns.
