patient found having syncopal episode after Shingrix shot. patient helped to seat by staff, responsive @ 11:20am, this RN called MD stat @ 11:21. RN noted pt breathing with grunting breath sound with small coffee ground emesis around comer of mouth, VS 143/97 HR 68 on RA 100% @ 11:23. MD stat team arrived @ 11:24am. SBAR report given to MD and RN. Care handed over. Patient was lethargic after taken over and 911 was call, Engine arrived @ 11:31am

Site: Pain at Injection Site-Mild, Systemic: Dizziness / Lightheadness-Severe, Systemic: Disoriented/Couldn't walk/no balance-Severe, Systemic: Shakiness-Severe, Systemic: Weakness-Severe, Additional Details: Patient got 2 vaccines (shingrix & flu) on 9/10/22 and felt fine. later on that day he got dizzy and felt disoriented, couldn't walk straight, off balance. The next day 9/11/22 he went to ER bc he thought he was having a stroke and got admitted into the hospital. Patient stated he still does not feel the same and seems like he wants compensation. He wants corporate to reach out to him.

Symptomtext

Rash at the injection site; redness at Injection site; warm to touch, 4 by 5 inches; Headache; Blurred vision; This case was reported by a consumer via call center representative and described the occurrence of injection site rash in a 60-year-old female patient who received Herpes zoster (Shingrix) (batch number 4NY45, expiry date 15th September 2023) for prophylaxis. Co-suspect products included Flu Seasonal QIV Dresden (Fluarix Quadrivalent 2022-2023 season) (batch number Y9JS7, expiry date 30th June 2023) for prophylaxis and flu seasonal qiv dresden pre-filled syringe device (Fluarix Tetra Pre-Filled Syringe Device) injection syringe for prophylaxis. Previously administered products included Zostavax (received on an unknown date). On 20th October 2022, the patient received Shingrix, the 1st dose of Fluarix Quadrivalent 2022-2023 season (intramuscular) and Fluarix Tetra Pre-Filled Syringe Device. In October 2022, less than a week after receiving Shingrix, Fluarix Quadrivalent 2022-2023 season and Fluarix Tetra Pre-Filled Syringe Device, the patient experienced injection site erythema and injection site warmth. On 20th October 2022, the patient experienced headache. On 21st October 2022, the patient experienced blurred vision. On 24th October 2022, the patient experienced injection site rash. The patient was treated with topical steroid nos (Steroid Cream) and antibiotics nos (Oral Antibiotic (Drug Name Unknown)). On an unknown date, the outcome of the injection site rash was not recovered/not resolved and the outcome of the injection site erythema and injection site warmth were unknown and the outcome of the headache and blurred vision were recovered/resolved. It was unknown if the reporter considered the injection site rash, injection site erythema, injection site warmth, headache and blurred vision to be related to Shingrix, Fluarix Quadrivalent 2022-2023 season and Fluarix Tetra Pre-Filled Syringe Device. This report is made by GSK without prejudice and does not imply any admission or liability for the incident or its consequences. Additional Information: GSK Receipt Date: 26-OCT-2022 Reporter's comment: The case was self-reported by the patient. The patient received a dose of Shingrix and Fluarix into left deltoid and experienced headache immediately. The headache resolved after 3 days. The day after vaccination, the patient experienced blurred vision that resolved after 2 days. Four days after vaccination, the patient developed a rash at the injection site. The rash was red, warm to touch and about 4 inches by 5 inches. The borders of the rash were well-defined and rectangular, not curved. The patient's doctor initially prescribed a steroid cream to treat the rash. He then prescribed an oral antibiotic because the rash was warm to the touch. No other details were reported. The reporter consented to follow up.

Discomfort Injection site reaction Swelling

Symptomtext

Patient called the pharmacy to report swelling and injection site reaction on the arm with the vaccines. Had patient check for temperature, which was 98.1 degrees Farenheit. Advised patient to put ice on the area to minimize swelling and rotate arm to minimize discomfort. Called patient 2 other times during the day to make sure she was doing well. Patient reporting doing better and feeling better.

Shingrix only adjuvant; Received only the adjuvant; This case was reported by a nurse via call center representative and described the occurrence of inappropriate preparation of medication in a 56-year-old female patient who received Herpes zoster (Shingrix) (batch number 4NY45, expiry date 15th September 2023) for prophylaxis. On 16th September 2022, the patient received Shingrix. On 16th September 2022, unknown after receiving Shingrix, the patient experienced inappropriate preparation of medication and inappropriate dose of vaccine administered. On an unknown date, the outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were unknown. Additional Information: GSK receipt date: 20-Sept-2022 Reporter's comment: The reporter consented to follow-up. Additional supportive information: The patient received only the adjuvant AS01B of shingrix, which led to inappropriate preparation of medication and inappropriate dose of vaccine administered.

Symptomtext

Patient presented to pharmacy on 7/24 and showed a left arm that was swollen and red all the way down to the left elbow and all the way around the arm. Patient stated that he had feelings of numbness in his left hand the night before and his whole arm was very hot to the touch. He was instructed to take Benadryl and seek out medical help if the condition got any worse.
