it was administered just the adjuvant; it was administered just the adjuvant and then the correct reconstituted dose; This case was reported by a nurse via call center representative and described the occurrence of inappropriate preparation of medication in a 61-year-old male patient who received Herpes zoster (Shingrix) (batch number 57ljn, expiry date 14th April 2023) for prophylaxis. On 30th September 2021, the patient received the 2nd dose of Shingrix. On 30th September 2021, unknown after receiving Shingrix, the patient experienced inappropriate preparation of medication and inappropriate dose of vaccine administered. On an unknown date, the outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were unknown. Additional details were provided as follows: The patient was inadvertently administered just the adjuvant alone of shingrix, which led to inappropriate preparation of medication and inappropriate dose of vaccine administered. The patient wanted to know what was in the adjuvant and was it safe. The health care professional mentioned that, correct reconstituted dose was already administered immediately after error. The reporter consented to follow up.
