Symptomtext

Thru interpreter, pt indicated initially swelling to rt arm radiating up into rt side of clavicle region, no fever or redness. Reported numbness and tingling sensation to rt arm, able to open and close hand, limited range of motion to rt arm. The vaccine given in rt deltoid on 12/14/2022. On visit, 2/8/2023, no redness, fever noted, pt provided details of incident.. Swelling noted rt upper arm with small amount to clavicular region. Continued c/o pain, limited range of motion, reports continued numbness and tingling to hand and fingers. grip strength intact. Rt arm remains warm and WNL color. Pt reports has been using otc meds for pain, cool compresses to arm. Informed client to follow up with private MD for further evaluation. Medical doctor notified of previous incident..

Symptomtext

The patient received his Shingrix vaccine on -1/30/23. There was no initial reaction. However, the patient said that 3 weeks after getting his vaccine he began to have ankle weakness/pain in ligaments. It peaked 1-1.5 months later with general muscle weakness. It spread to his shoulders, hips, knees tendons locking up/snapping. He felt arthritis pain and general muscle weakness. He went to his PCP in April 2023. They told him not to get the booster of Shingrix. He stated that he did have a prior reaction to the flu vaccine with ligament/muscle weakness many years ago. The Dr is taking blood and testing him for inflammatory markers.

Pyrexia Rash

Symptomtext

Patient called and stated she had a rash that she had tried to treat with benadryl. the rash did not improve and in fact got worse. The rash was in her hair and over her body. She also had a fever of 101.5 and was concerned. I asked her if anything had recently changed (medication, detergent, deodorant, etc) and she said no. I asked her if she had any breathing problems and she said not yet. I suggested she get checked out by a doctor since it had been 11 days and it was getting worse even though she was treating it.

Symptomtext

Patient received vaccine on 1/10/23. Patient reported symptoms to pharmacy a couple of days later on 1/12/23 describing signs of possible allergic reaction. He didn't tell us until today 1/16/23 that he had to go to dermatologist. Patient had redness, swelling, skin peeling, and itching of his face and head. Patient went to dermatologist office for follow up and assessment. Dermatologist prescribed steroid cream to help with redness and itching. Patient states they are getting better and symptoms are improving at this time.

Morning following vaccine patient woke up feeling weak with severe nausea and vomiting. Fever continued to go up. Could not eat or drink all day. Significant swelling and soreness around injection site with rash. Was able to start eating very little the following day but is still weak from the event as of today (11/23/2022).

Expired vaccine administered; This non-serious case was reported by a pharmacist via call center representative and described the occurrence of expired vaccine used in a 75-year-old male patient who received Herpes zoster (Shingrix) (batch number 5NC4N, expiry date 08-OCT-2023) for prophylaxis. On 07-NOV-2023, the patient received Shingrix. On 07-NOV-2023, an unknown time after receiving Shingrix, the patient experienced expired vaccine used (Verbatim: Expired vaccine administered). The outcome of the expired vaccine used was unknown. patient was administered a dose of Shingrix on 07Nov2023; This non-serious case was reported by a pharmacist via call center representative and described the occurrence of expired vaccine used in a 70-year-old male patient who received Herpes zoster (Shingrix) (batch number 5NC4N, expiry date 08-OCT-2023) for prophylaxis. On 07-NOV-2023, the patient received Shingrix. On 07-NOV-2023, an unknown time after receiving Shingrix, the patient experienced expired vaccine used (Verbatim: patient was administered a dose of Shingrix on 07Nov2023). The outcome of the expired vaccine used was unknown. Administered only the diluent; Administered only the diluent; This case was reported by a other health professional via call center representative and described the occurrence of inappropriate preparation of medication in a 60-year-old female patient who received Herpes zoster (Shingrix) (batch number 5NC4N, expiry date 8th October 2023) for prophylaxis. On 30th January 2023, the patient received the 1st dose of Shingrix. On 30th January 2023, unknown after receiving Shingrix, the patient experienced inappropriate preparation of medication and inappropriate dose of vaccine administered. On an unknown date, the outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were unknown. Additional Information: GSK receipt date: 20-FEB-2023 Reporter's comment: The HCP(health care professional) reported that patient received dose of Shingrix in the series was administered with diluent only. No side effects was reported. The reporter consent to follow up. Additional Supportive Information: The patients received Shingrix vaccines with diluent only which led to inappropriate preparation of medication and inappropriate dose of vaccine administered.
