Symptomtext

Patient presented today requesting COVID-19 booster dose. The immunization screening questions were reviewed. Booster dose given in accordance with Health Officer guidance at 8:28am. Patient requested flu vaccine. Administered at 8:27am. Shortly after at about 8:29am patient stood up still in nurse vaccine station and stated he felt dizzy, and he needed to lie down. PHN gently helped guide patient to the floor, into the supine position. Patient went unconscious immediately and PHN began to hear a gurgling noise, PHN took off his face mask, checked his airway nothing was found, rolled him onto his right side, PHN helped and lifted his legs. Patient was out for about 15 seconds. PHN then placed him back onto his back and he began to talk. He then went out again and made the gurgling noise, his face turned red. PHN assessed airway, nothing found rolled patient onto his right side. PHN continued to hold his legs up, he was out for approximately 15 seconds again. Patient returned to conscious level and was placed back onto his back, he began talking to the nurses and stated he has a history of vasovagal episodes, patient was encouraged to take a few deep breaths. PHN asked if he had eaten before coming in today, he stated he had not. He was given a cookie and a juice, he had two sips of the juice. The ambulance was called at 8:30am by site lead. PHN notified patient that the ambulance was on the way, vital signs were taken B/P 120/70, P86, O2 Sat 99% RA, he remained lying on the floor until the ambulance arrived at 8:50m. He continued to have trouble with orthostatic hypotension, the ambulance crew took him to the hospital.

Clinician spoke with patient by phone on 12/20/21 and patient reported the following: Started to experience pain in left arm on 12/17 after getting the vaccines done. On Saturday started to get pain in left axillary area and redness in skin overlying axilla. Having pain that comes and goes in that area over last 2 days that is severe enough that hasn't been able to work. No weakness in fingers. Able to move hand normally. No fever. Used ibuprofen 400 mg yesterday am with some improvement. Reviewed 2 pictures patient sent to mobile phone showing mild pink erythema overlying left axilla. Appears that having reaction with some lymphadenopathy and some soft tissue swelling and skin erythema, all of which have been reported as potential of side effects of flu vaccine or Tdap vaccine. Local swelling can also happen with pneumovax. Advised patient to use ibuprofen OTC up to 800 mg every 8 hours for next few days. Call if symptoms don't improve over next 3-4 days. Call if any loss of strength or function in hand or fingers Pt had unrelated appointment 12/23/21 and no mention is made about continued s/s from injection reaction.

Symptomtext

On 4/19/2022, at 9:02 am, flu vaccine administered to patient, despite having received one October 2021. He was monitored for 15 minutes after receiving both the flu and Moderna vaccines. No signs of distress observed or reported. He was provided with education on what to expect, adverse effects, and when to seek care (both verbal and written) on both the flu and COVID-19 vaccine. Reviewed dates of flu season with all staff, and how to better view the records in the system. It was found most staff were using the wrong page, which doesn't reveal as easily which vaccines are due versus completed. Provided instructions to contracted staff to double check.

Per staff report, the September dose in database was not observed due to the most recent administration being at the bottom of the record. Pt was monitored for 15 minutes after receiving both the flu and Moderna vaccines. No signs of distress were observed or reported. Pt was provided with education on what to expect, adverse effects, and when to seek care ? both verbal and written ? on both the Flu and COVID-19 vaccine.

Patient was administered his 11 year vaccines (tdap, mcv4, hpv) on 5/12/2021 and had an appointment yesterday for his second hpv. Medical assistant says she checked and thought patient did not have his 11 year vaccines yet. Patient was administered his 11 year vaccines again on 3/15/2022 (tdap, mcv, hpv#2 and flu).

Symptomtext

Notes: 3 day history of swelling of right hand, right arm after receiving flu vaccine. Reassuring clinical examination. Of note, brother has a much more significant swelling of his arm and hand after vaccination. Given history of emesis and severe swelling in brother, will give Rx for epipen today Pt has received flulaval in recent years without problems will give trial of benadryl. watch for worsening swelling, rash, worsening emesis or diarrha

Family arrived to office with 2 children (ages 2yr and 4mo) for well child visits. The mom asked for flu vaccine for the 2yr old and nurse misunderstood and thought that request was for the 4mo old. Vaccine given erroneously to the 4mos old patient. Provider has discussed incident with family.
