Dyspnoea Headache Hypersensitivity Malaise

Symptomtext

Per patient report, at unknown time the same day as vaccination, began feeling "sick" and developed a headache. Later felt shortness of breath and was seen in the emergency room and diagnosed with an allergic reaction. Her son was vaccinated on the same day and had the same symptoms and also was diagnosed as having an allergic reaction. No medication were received in the ER. Discharged home the same day. Received report from patient one month after event occurred.

Engerix-b expired administered; This case was reported by a other health professional via call center representative and described the occurrence of expired vaccine used in a 51-year-old female patient who received HBV (Engerix B) (batch number 5NY2K, expiry date 29th May 2022) for prophylaxis. Co-suspect products included hepatitis B vaccine pre-filled syringe device (Engerix B Pre-Filled Syringe Device) injection syringe for prophylaxis. On 8th June 2022 16:30, the patient received Engerix B. On 8th June 2022, the patient started Engerix B Pre-Filled Syringe Device. On 8th June 2022 16:30, unknown after receiving Engerix B and Engerix B Pre-Filled Syringe Device, the patient experienced expired vaccine used. On an unknown date, the outcome of the expired vaccine used was unknown. This report is made by GSK without prejudice and does not imply any admission or liability for the incident or its consequences. Additional Information: GSK Receipt Date: 08-JUN-2022 Reporter's Comment: The reporter reported that, the patient was administered with expired dose of Engerix-B. No reactions were reported. Symptomtext

Administrated an adult dose of Engerix-B to a 17 years old; Administrated an adult dose of Engerix-B to a 17 years old; This case was reported by a pharmacist via call center representative and described the occurrence of adult product administered to child in a 17-year-old male patient who received HBV (Engerix B adult) (batch number 5NY2K, expiry date 29th May 2022) for prophylaxis. On 26th February 2021, the patient received the 1st dose of Engerix B. On 26th February 2021, unknown after receiving Engerix B, the patient experienced adult product administered to child and overdose. On an unknown date, the outcome of the adult product administered to child and overdose were unknown. Additional details were provided as follows: An adult dose of Engerix-B was administered to a 17 years old male patient which led to adult product administered to child and overdose. No adverse event was reported. The reporter did not consented to follow up. Upon internal review, following corrections were performed on 23rd April 2021: The exclude from pader classification from general tab was removed. The suspect was updated to Engerix B adult.
