Symptomtext

Expired Rotarix given; This case was reported by a nurse via call center representative and described the occurrence of expired vaccine used in a 4-month-old male patient who received Rota (Rotarix lyophilized formulation) (batch number 743NB, expiry date 11th July 2022) for prophylaxis. Co-suspect products included rotavirus vaccine oral applicator device (Rotarix Oral Applicator Device) oral applicator for prophylaxis. On 4th August 2022, the patient received Rotarix lyophilized formulation and Rotarix Oral Applicator Device. On 4th August 2022, unknown after receiving Rotarix lyophilized formulation and Rotarix Oral Applicator Device, the patient experienced expired vaccine used. On an unknown date, the outcome of the expired vaccine used was unknown. This report is made by GSK without prejudice and does not imply any admission or liability for the incident or its consequences. Additional Information: GSK Receipt Date: 04-AUG-2022 Reporter's Comments: The patient was inadvertently given an expired dose of Rotarix. The reporter consented to follow up from safety. Additional Supportive Information: The patient received Rotarix which was already expired, which led to expired vaccine used.

Symptomtext

The Hepatitis B Vaccine with the Pediarix would have been 2nd Dose and I believe should be okay. The child was seen for a Well Child Visit and should have been seen for a 2 week old well child since he was 3 weeks and 5 days at the time of the visit however the Receptionists and Nurse had him scheduled for the 2 month Well Child so the vaccines were given at this visit. This was recently discovered by another Provider at the child's 9 month well child visit. The child did NOT experience any ill effects from the vaccine or Mom did not report any. My question is should the child be vaccinated since they were not at least 6 weeks old when received the first set of vaccine.
