Symptomtext

Patient had bloody stool, jelly like consistency. Patient was seen in primary care clinic and then sent to hospital. Pt was diagnosed with intussusception. Pt had two air enemas were performed for reduction. Patient was monitored in ED for four hours post reduction and discharged home.

Symptomtext

Patient came in for 15 month WCC on 8/31/23. Already had 15 month on 7/10/23. Received HIB and Hep A vaccines at both visits in error. Patient did not report any symptoms in the weeks following and at the 18 months WCC mom stated no concerns in development.
