On 31-OCT-2023, the patient received Arexvy. On 31-OCT-2023, an unknown time after receiving Arexvy, the patient experienced inappropriate preparation of medication (Verbatim: 2 diluent of Arexvy was mixed together and was administered to a patient. lot# and expiration same for both diluent.) and inappropriate dose of vaccine administered (Verbatim: 2 diluent of Arexvy was mixed together and was administered to a patient. lot# and expiration same for both diluent.).
