Symptomtext

Spoke with client's mom today 8/24/2021 at 9:40am, she states that the client developed a rash shortly after receiving vaccines from me yesterday. It was red and itchy from the belly button to his groin area. She put an antibiotic cream on the affected area and the rash appeared to go away. This morning when the client woke up, he had a rash covering his back and the client states that his body is broken because both of his legs hurt. I advised mom to call his pediatrician and let him know what is going on and whether he will be able to receive future doses of these vaccines. I also told mom she could try Benedryl on his rash to see if that helps. Mom states that otherwise the client feels fine- no fever or anaphylaxis.

Symptomtext

2-3 minutes after the shots were givenm mom approached my assistant because of change in baby's color. Wfen I came in a few seconds later, baby is awake,limp, pale, dusky but not cyanotic, breathing, clearbreath sounds, heart rate 100/min. , regular in rhythm, no seizure movement and afebrile. Stimulated baby to cry by patting feet and about 1-2 minutes later, he pinked up,heart rate went up to 180's (crying) and 02 sat was 98%. Baby was observed in the room was observed x 1 hour before he was sent home, was feeding well. A phone follow-up was done in the afternoon and per dad, baby was fine. Anothe phone call was done the following day and per dad, baby had a fever and was advised to give acetaminophen.

Expired Pediarix administered; 1st or 2nd dose administered to a 6 months-old; This case was reported by a consumer via call center representative and described the occurrence of expired vaccine used in a 6-month-old male patient who received DTPa-HBV-IPV (Pediarix) (batch number 934NJ, expiry date 10th December 2021) for prophylaxis. Co-suspect products included dtpa-hbv-ipv vaccine pre-filled syringe device (Pediarix Pre-Filled Syringe Device) injection syringe for prophylaxis. On 15th December 2021, the patient received Pediarix and Pediarix Pre-Filled Syringe Device. On 15th December 2021, unknown after receiving Pediarix and Pediarix Pre-Filled Syringe Device, the patient experienced expired vaccine used and inappropriate schedule of vaccine administered. On an unknown date, the outcome of the expired vaccine used and inappropriate schedule of vaccine administered were unknown. This report is made by GSK without prejudice and does not imply any admission or liability for the incident or its consequences. Additional Information: GSK receipt date: 9-Feb-2023 Reporter's comment: The patient's mother mentioned that she thought this was the 1st or 2nd dose that the patient had received, but she was not sure, as the dose schedule was at 2, 4 and 6 months of age. The reporter also did not had the name of the product administered only the lot and she thought was administered in the patient 's right thigh muscle, but she was not sure. The reporter consented to follow-up. Additional supportive information: The patient received an expired dose of Pediarix followed inappropriate schedule, which led to expired dose used and inappropriate schedule of vaccine administered.
