Nausea Vomiting

Symptomtext

Client became nauseous and vomited about 5 minutes after all vaccines administered. Only 1 vomiting episode and nausea lasted about 10 minutes. Client vital signs were taken and client remained seated in a chair. All vital signs were stable and client verbalized feeling ok and back to normal about 10 minutes after incident began.

Granuloma annulare Rash

Symptomtext

I came out with rash type lumps all over my body in numerous places 9 days after I had the vaccine. I ignored this thinking it was a bite as that is what they looked like. They grew to large lumps all over my body. I reported this and the dermatologist said it is granuloma annular. I have had adverse reactions to vaccinations in the past such as seizures and sickness etc.

Patient was administered a dose on 17Jan2022 that had expired 01Jan2022; This case was reported by a other health professional via call center representative and described the occurrence of expired vaccine used in a 18-year-old female patient who received HBV (Engerix B) (batch number 95SN2, expiry date 1st January 2022) for prophylaxis. Co-suspect products included hepatitis B vaccine pre-filled syringe device (Engerix B Pre-Filled Syringe Device) injection syringe for prophylaxis. On 17th January 2022, the patient received the 2nd dose of Engerix B and Engerix B Pre-Filled Syringe Device. On 17th January 2022, unknown after receiving Engerix B and Engerix B Pre-Filled Syringe Device, the patient experienced expired vaccine used. On an unknown date, the outcome of the expired vaccine used was unknown. This report is made by GSK without prejudice and does not imply any admission or liability for the incident or its consequences. Additional details were provided as follows: The age at vaccination was not reported. The patient was administered a dose of Engerix-B on 17th January 2022 that had expired 1st January 2022, which led to expired vaccine used. The reporter consented to follow up.

On 5th January 2022, unknown after receiving Engerix B pediatric and Engerix B Pre-Filled Syringe Device, the patient experienced expired vaccine used. On an unknown date, the outcome of the expired vaccine used was unknown. This report is made by GSK without prejudice and does not imply any admission or liability for the incident or its consequences. Additional details were provided as follows: Medical assistant reported that a patient received a dose of pediatric Engerix B that had already expired on 1st January 2022, which led to expired vaccine used. The reporter did not consent to follow up. The vaccine administration facility was same as primary reporter.

received Pediarix and Engerix-B pediatric dose at the same time; This case was reported by a other health professional via call center representative and described the occurrence of accidental overdose in a 2-month-old female patient who received HBV (Engerix B) (batch number 95SN2, expiry date 1st January 2022) for prophylaxis. Co-suspect products included hepatitis B vaccine pre-filled syringe device (Engerix B Pre-Filled Syringe Device) injection syringe for prophylaxis, DTPa-HBV-IPV (Pediarix) (batch number 7P2Y3, expiry date 8th April 2023) for prophylaxis and dtpa-hbv-ipv vaccine pre-filled syringe device (Pediarix Pre-Filled Syringe Device) injection syringe for prophylaxis. On 8th December 2021, the patient received Engerix B 10 mcg, Engerix B Pre-Filled Syringe Device, Pediarix and Pediarix Pre-Filled Syringe Device. On 8th December 2021, unknown after receiving Engerix B, Engerix B Pre-Filled Syringe Device, Pediarix and Pediarix Pre-Filled Syringe Device, the patient experienced accidental overdose. On an unknown date, the outcome of the accidental overdose was unknown. This report is made by GSK without prejudice and does not imply any admission or liability for the incident or its consequences. Additional details were provided as follows: The patient received Pediarix and Engerix B pediatric dose at the same time yesterday, which led to accidental overdose. The reporter consented to follow up.
