Symptomtext

Moderna COVID-19 Vaccine EUA. Received Moderna booster dose on 12/29/2022 and on 1/09/2022 experienced severe hives all over her body requiring her to seek medical care at ED. She was given IV Benadryl, Pepcid, and steroid. This medication was taken for 5 days with not much relief. The hives come in waves and has continued since 1/09/2022. Seen allergist and was placed on Singular, Pepcid, Zyrtec, Xyzal, Prednisone, and Benadryl. As of today, 1/14/2022 is still having the waves of hives all over her body that are itchy and reddened. Also feels like there is a lump in her throat at times, but has never caused difficulty breathing and was never given epinephrine. Plans to continue current medications and follow up with Allergy doctor.

Symptomtext

Patient notified office today reporting discomfort to left arm since she received the vaccine. "When I'm not doing anything it's a little sore, my arm feels bigger than normal. If I have to move it for anything it hurts like crazy, feels like a horrible bruise from the whole top of my left arm down to my elbow at times." Denies any redness or warmth to site.

Muscle twitching Nausea Unresponsive to stimuli

Symptomtext

Child sitting on dad's knee, leaning against him; seconds after injection, child slumped to right side, was unresponsive. Called child's name, shook shoulder; placed him on floor. Respirations continued. He was unresponsive for 40-45 seconds; eyes fluttered and his body twitched several times; not indicative of seizure but tonic/clonic muscle twitching. No evidence of seizure activity but slight hesitation when answering questions. Limited neuro exam preformed; vitals started with oxygen at 1L by non-rebreather mask; legs elevated on chair. Alert and oriented times 3; no loss of bowel or bladder. Vitals taken every 1-5 minutes for next hour remained stable. When he sat up he experienced slight nausea; crackers, juice and water provided. Oxygen removed after 7 minutes; saturation level remained stable at 97% on room air. When he was moved from floor to chair after 18 minutes he experienced nausea increased; cold pack placed on neck, wrist pulse points, forehead.

received Boostrix (tdap) on 10/15/2020 instead of DTaP.; 2 years ago received Boostrix (tdap) on 10/15/2020 instead of DTaP; This case was reported by a nurse via call center representative and described the occurrence of wrong vaccine administered in a 4-year-old male patient who received DTPa (Reduced antigen) (Boostrix) (batch number 9AN49, expiry date 25th March 2022) for prophylaxis. On 15th October 2020, the patient received Boostrix. On 15th October 2020, unknown after receiving Boostrix, the patient experienced wrong vaccine administered and inappropriate age at vaccine administration. On an unknown date, the outcome of the wrong vaccine administered and inappropriate age at vaccine administration were unknown. Additional Information: GSK receipt date - 31-Aug-2022 Reporters comment: The reporter reported that 2 years ago patient received Boostrix (tdap) instead of DTaP vaccine. The reporter stated that did not know if patient needs a Dtap. No additional details provided. The reporter wanted to know how to proceed with DTaP series. The reporter did not consent for follow-up. Additional Supportive information: The patient received Boostrix (tdap) instead of DTaP vaccine which led to wrong vaccine administered and inappropriate Age at vaccine administration

Administered the listed vaccines that were due on 6/10/2021. Upon chart auditing noted that client's record that had been under the name and had additional vaccines added to it which invalidated several vaccines given on 06/10/2021. The client is in therapy and upon calling the client's medical case manager she reported she did not correctly report the client's name at the appointment on 06/10/2021 and that the name had been corrected. Advised medical case manager that client had received additional doses of Tdap, Hep A, Hep B, Varicella, and Menactra since full shot record was not available at appointment in June. Medical case manager reported the client had no reactions after vaccine administration.
