Symptomtext

Patient started having emesis and bloody bowel movements 5/9/22, family called EMS 5/11/22 and patient was taken to the local hospital and found to have intussusception. She was transferred to Hospital where an attempt at air enema reduction was unsuccessful and patient required emergent surgery and bowel resection for incarceration ileocolic intussusception 5/11/2022.

Symptomtext

Patient started to have foul smelling loose stools and abd pain on 3/29/22, the day following her vaccines and noted to have streaks of blood on 3/30/22. Went to ED where had normal exam. Stool studies collected and reported + for Rotavirus. Patient had no sick contact, no travel and is not in daycare. Formula fed.

The wrong diluent was mixed with vaccine. Medical assistant used the wrong diluent - sterile diluent (sterile water) instead of the sterile solution (citrate bicarbonate buffer) when mixing the vaccine. Administered with to patient with sterile water
