Patient has needle anxiety. She seemed fine during the vaccine administration. After receiving all 4 vaccines, she had syncope and fell face down from the sitting position. We called 911 but patient refused to be taken to the ER. Patient's roommate picked her up.

Patient reported pain at the injection site almost immediately after vaccine was given at 4:50pm. About 5 minutes later 4:55pm the patient reported she didn't feel good. Upon entering the room where the patient was, she was shaking uncontrollably, having difficulty breathing, and very anxious. The patient reported her throat felt tight and she felt like she was going to crawl out of her skin. She also reported neck and back pain. The patient's blood pressure was 182/101 and her heart rate was 104. Patient was given 1mg of adrenaline at 5:00 pm. to counteract reaction and EMS were called. Ambulance arrived within 5 minutes of call and had to put patient on a stretcher as she could not stand and walk. Patient left the building at 5:20 p.m.

Started with having some heart palpitations roughly 3-4 hours after receiving the vaccines. Then with the 5-6 hour mark I started with the hives. Hives continue to get worse, swelling in my face and around my eyes. Treating with Benadryl and Claritin with next to no relief. PCP sent in methyl prednisone to start, 6 day taper of steroid., outcome still pending depending on how medication works. I work in my PCP's office so I they did not make me schedule an appointment at this. We are monitor symptom's at this time on the steroid.

Patient presented to the ED after experiencing shortness of breath on exertion. Patient was found to have BNP of 3163 and chest x ray showed bilateral pleural effusions. Echocardiography showed left ejection fraction of 10%. Cardiac catheterization showed nonischemic cardiomyopathy and moderate pulmontary hypertension. Patient was fitted for life vest and started on bumex, dapagliflozin, losartan and metoprolol. Patient underwent cardiac MRI which showed dilated cardiomyopathy.

Report per employee: Received Flu shot to rt arm 10/25 afternoon about 1330. Returned to office and took Advil. Approx. 1545-1600 started feeling itching to nares, eyes, mouth, scratchy throat. Took Benadryl and Hydrated Itching continued noted face and body rash by the time my shift ended 1745 Bathed with Aveno and Baking soda took 50 mg Benadryl and 25 mg Vistaril. Drank water with baking soda. Itching continues today few bumps on face, arms, back , topical Aveno lotion and Benadryl to skin. Took 25 mg Benadryl this AM On 10/27/23 called Employee Health to report increase in hives and joint pain. Declined to see Occupational Health MD and will use personal insurance.

The first day (10/9) patient was feeling sleepy and restless, and the next day (10/10) somewhere around 9 pm when the patient took a shower, noticed hives broke out everywhere and skin was very itchy. Patient went to a PCP the next day (10/11) and was given a course of treatment which can't be remembered.
