7/26/22 1:30 client(accompanied by mother) rec'd above immunizations. 1 min later c/o headaches, nausea, diaphoretic, color pale. Client assisted to exam table to lay down, legs elevated. BP 119/72, Pulse 72. Alert and oriented x3. Cool cloths to forehead, neck. 1:35-color improved, client engaging in conversation about school. no further c/o nausea, 1:45-Head elevated to semi-fowlers, cont to "I'm feeling better", Pulse 72 and regular. 1:55-continues to improve, no further complaints. color improved. Head elevated to fowler's position. 2:05-client and mother left facility in stable condition. 8/1/2022- mother called to report client was seen @ local ER on the evening following her immunizations. @ 11:30 she c/o face swelling, lips tight and throat tightening. "They said she had an anaphylactic reaction. The only thing that was different that day was the vaccines. Treated with epi. She will now carry an epi-pen with her. Client discharged home last that night.

Vasovagal response. Skin color became pale, white lips, but was still able to speak with family and nurses. Pt was shaking, diaphoretic, and rap pulse on assessment. Pt stated, "my ears started ringing, bad pain in my left arm, things got dark, and remembering sitting on the floor when I came to." 911 was called. After deep breathing exercises and cool compresses pt slowed breathing, pinks became pink, and blood pressure/HR came back in normal range for his age. O2 sats were 98%.

Syncopal event he has had this occur in the past with every vaccine- he was administered the vaccines while lying on the exam table- he did initially pass out- feel nauseous- seemed ok after 5 minutes and tried to sit up only to become dizzy again and need to lie down- he was given ice pack- ammonia inhalant used x 1- he had several attempts at sitting up but became dizzy and nauseous- needed to lie down- he tried to drink orange juice and then some gatorade- vomited a small amt after sleeping for 20 minutes he finally felt well enough to stand, walk and go home total time at least 60 minutes from vaccine administration until he was able to walk out the door

The patient was sitting on the examination table and the four vaccines were administered by the primary care, two in each arm. The last vaccine that was applied was HPV. One minute after the application, the patient fainted and fell to the floor. The patient fell on the left side of her body. She didn't hit her head. The patient regained consciousness a few seconds later. She was oriented in time, space and person. Patient was left on the floor for 2-3 minutes and was helped to move onto the stretcher. There were no more incidents. The neurological examination after the incident was normal. There was no evidence of significant trauma that could be seen on physical examination.

Pt. received her coivd vaccine, menactra, and tdap shot on 08/12/21. At 11:38 Am the Pt passed out for 30 seconds. pt. was very pale, diaphoretic, and stated she felt flushed. After the pt. came to the nurse took her vitals which were Bp92/60, HR97, O2 98%. The nurse had pt. lie down for a few minutes and gave her some food and water. At 11:57 Am the pt. passed out again for 30 seconds and came to. The nurse immediately laid the patient down and took her vitals. BP98/70, HR 53, O2 99%. Pt. was pale, diaphoretic and felt flushed. The nurse offered her more food and water and retook the vitals at 12:07 Pm Bp98/70, HR90, O2 98%. Pt. was able to leave at 12:44 Pm. F/U 8/13/21 no subsequent events, pt. was fine.

Client is a 11 year old female that presented to the immunization clinic for Tdap, HPV9, and MCV4 vaccine. Client was called back to vaccination area around 0930. Client reported no vasovagal hx-confirmed w/ mother. This writer administered Tdap vaccine & client began to lean over in chair. This writer & mother supported client. Client reported feeling "nauseous & tired." This writer explained to client that she had experienced a vasovagal reaction. Client was able to tell this writer name, DOB, and location. This writer asked co workers for assistance. Co-worker obtained ice pack for client because she reported feeling hot. This writer obtained trash can (reported feeling nauseous) & began to fan the client. This writer noted client's lips/ skin were pale. Client reported eating & drinking this morning prior to appointment. Client reported feeling "slightly better." Client was transported to bed in clinic area by nursing staff (co-workers & this writer). Client was provided water & saltine crackers. Client was encouraged to raise legs to increase blood flow. This writer noted client's lips/skin returned to normal color. Client reported feeling better & stated "I am ready for the next vaccines." This writer explained that client would need to remain lying down for next two injections. Client and mother verbalized understanding. This writer administered MCV4 & HPV9 with no issues. Client was encouraged to stay lying down for 15 minute wait. Client waited 15 minutes & reported feeling "good." Client & mother were observed walking out of clinic @ 1015. This writer encouraged mother to inform future vaccinators of vasovagal hx of client, so vaccines can be given lying down.

She had an LOC and a tonic seizure . The office code for such an emergency was called, patients vitals were measured, she became responsive and alert within a few minutes, was oriented to place and person. She was able to go home without any further incidence or complications

After vaccines patient and mother stayed in the room for 3 to 4 minutes, then walked out to the hallway towards exit door, but patient fell unconcious hit the head against the edge and counter wall. Pt had a 10 sec seizure and within 30 mins became alert, responsive and not sleepy or drowsy. Pt was taken in a wheelchair back to room and after evaluation - laceration was noticed on scalp. Laceration was treated and successfuly the bleed was stopped. BP was retaken 149/90. Ems was called and patient was taken to the ER for further evaluation.

Shortly after vaccines were given, Patient's eyes rolled back in her head. Mom laid her down on the table and she passed out. She had a brief tonic clonic seizure that involved all 4 extremities and lasted a few seconds. By the time the physician entered the room, she was awake and crying. Within 15 minutes, Patient was able to walk out of the office with her mom.

Patient fainted in chair approximately 1-2 minutes after receiving Tdap vaccination--nurse had water bottle in cabinet and quickly sprinkled water over him while he slumped in chair against wall; nurse also applied wet & cold paper towels to patient's face/neck/arms. Patient regained consciousness after a couple of minutes. At no time did patient fall or strike any object or any part of the room/furniture. Cold pack was applied to patient's face/neck/arms, and patient ate lollipop. After ~25 minutes, nurse walked patient to waiting room to wait until he felt able to leave on his own--RN

Hives covering legs and on wrist and lower abdomen. Hives began within a week of injection and continued to spread for a week. Began applying traimcinolone 2 weeks after injection on the affected areas. Began taking 180mg of fexofenadine 2x per day, starting 15 days after the injection. Spreading has stopped, but current hives are still swollen and itching.

47 y/o female reports a reaction after receiving the TDAP vaccination on 5/27/2021 . No prior reaction to immunization in the past. She reports a onset of pain and swelling at the injection site (left deltoid area) that worsened during the day on (Thursday) followed by a generalized frontal headache , nausea but no vomiting, and hives( starting in left arm, that radiated to left hand and jawline, chills. No shortness of breath. She developed a fever which lasted three days: on Friday 5/28/21 , temp 101.0 and 100.8-100.1 on Saturday 5/29/21 .On Sunday 5/31/21 temp was 99.9. she later took Diclofenac for pain with minimal improvement. Tuesday 6/1/21 She is afebrile temp 98.7 and still c/o soreness and swelling at site but feels symptoms are resolving. Advised her to come in to site today to be assessed. She is sensitive to tactile stimuli. No redness or warmth at the injection site site. FNP

Expired Boostrix administered; This case was reported by a other health professional via call center representative and described the occurrence of expired vaccine used in a 34-year-old male patient who received DTPa (Reduced antigen) (Boostrix) (batch number CM7KN, expiry date 29th January 2023) for prophylaxis. Co-suspect products included dtpa vaccine. pre-filled syringe device (Boostrix Pre-Filled Syringe Device) injection syringe for prophylaxis. On 20th February 2023, the patient received Boostrix and Boostrix Pre-Filled Syringe Device. On 20th February 2023, unknown after receiving Boostrix and Boostrix Pre-Filled Syringe Device, the patient experienced expired vaccine used. On an unknown date, the outcome of the expired vaccine used was unknown. Noticed missing hair just below and to the right of the crown of head about the size of a quarter on 7/29/2021. Checked regularly and about 8/17/2021 noticed the size of the missing hair had grown to about the size of a half-dollar. The following day 8/18/2021 noticed substantial chunk of hair dandling from left temple. Gently tugged and it came out with any force. Scheduled dermatologist appointment for as soon as possible to determine official diagnosis, treatment and expected outcomes. Soonest available times are weeks out (8/27/2021). Suspect this could be Alopecia Areata.

Father completed paperwork indicating client was 12 yrs of age, correct DOB 9/21/2009 given also at time of registration. Nurse preparing vaccine noted age as 12 as indicated by father and did not calculate age using DOB. Pfizer COVID vaccine offered to parent and accepted. COVID and other vaccines administered. Client returned with parent at appropriate time for second dose. Nurse administering vaccine at that time noted age was 11 yrs. and vaccine not given and why. Mother contacted office the following day to discuss COVID being given. Mother informed according to current CDC guidelines, dose # 1 was valid and that second dose should be given when the client turns 12 yrs of age. Mother also told that if CDC guidance changes she would be contacted. Mom verbalized understanding

received a dose of Boostrix instead of a dose of Infanrix; received a dose of Boostrix / DOB 08-DEC-2014; This case was reported by a nurse via call center representative and described the occurrence of accidental underdose in a 6-year-old male patient who received DTPa (Reduced antigen) (Boostrix) (batch number CM7KN, expiry date unknown) for prophylaxis. On 14th April 2021, the patient received Boostrix (intramuscular). On 14th April 2021, unknown after receiving Boostrix, the patient experienced accidental underdose and inappropriate age at vaccine administration. On an unknown date, the outcome of the accidental underdose and inappropriate age at vaccine administration were unknown. Additional details were provided as follows: The nurse reported that 6 year old patient received a dose of Boostrix instead of a dose of Infanrix vaccine, which led to accidental underdose and inappropriate age at vaccine administration. The reporter consented to follow up. The reporter did not have the expiration date for the product. No further information provided.
