Symptomtext

Patient was walking around the store after the shot. He wasn't feeling well within about 10 minutes so sat down and then fainted. He came to quickly, but then kept fainting maybe 3 times. He was extremely pale/gray and sweating profusely. He was very groggy whenever he came to. We called the paramedics with in a few minutes of him fainting initially.

At work while sitting at my desk I became dizzy, broke out in sweat, disoriented, blurred vision, slurred speech. Called plant nurse. She said my face was drooping, I couldn't smile or stand. She called Ambulance. Ambulance arrived and decided symptoms of stroke and called for helicopter to take me to medical center. When I arrived they did CT and I was later told it showed possible areas of no oxygen to brain. They gave me TPA drug and almost immediately I could see improvement - my headache cleared. Throughout the next couple hours I regained movement on right side and all functionality returned except weakness in right arm. I stayed in ICU for 24 hours as required after receiving TPA and while additional tests were ran then was discharged on Friday, April 16, 2021

ED to Hosp-Admission Discharged 8/27/2021 - 9/1/2021 (5 days) Hospital Patient presents with ? Shortness of Breath ? Generalized Body Aches 1. COVID-19 viral infection?patient has been having diarrhea, generalized body aches fever and cough. He is tested positive for COVID-19. He was actually vaccinated 2 months ago. Patient's oxygen saturation is fairly normal. Patient will be admitted and given IV dexamethasone secondary to dyspnea. He does not need to remdesivir or convalescent plasma at the moment. His x-ray is somewhat atypical and that he has left basilar infiltrate. However his procalcitonin level is also quite low. We will hold off on antibiotics for now. 2. Diabetes mellitus type 2?continue with insulin protocol. 3. BPH?continue with tamsulosin and finasteride. 4. COPD?patient has a significant wheezes. There is likely been exacerbated by his COVID-19 infection. He will be on steroids.

Patient had anxiety on arrival to the Covid vaccine clinic. Went into rapid atrial fibrillation on arrival due to anxiety. This occurred prior to receiving the vaccine. Patient recovered on her own and later received the vaccine. She then went back into rapid Afib and was transferred to the Emergency Department. Patient has a long history of A fib. Does not appear to be vaccine related.

received the Johnson and Johnson covid vaccine stayed at the health department for 30 min due to the anaphylactic reaction from the flu vaccine. went home. started having head to toe redness and itching. contacted the health department that gave me the vaccine to see if this was a normal reaction. started developing hives. took 25mg Benadryl. heath department advised me to contact my pcp. contacted my pcp. Nurse advised me to also take an additional 25mg Benadryl, 20mg Pepcid, and 10mg Claritin until hives and itching stopped but if symptoms worsened to go to the emergency room. stayed on the additional allergy medications for 4 days until hives where gone and itching was tolerable.

Patient noted to be having some reaction symptoms shortly after getting the COVID vaccine. Vaccine given at 1327 - spouse noted to have given her Benadryl 25 mg at 1338 when she first reported HA and numbness starting on the R side of her body. Patient was assessed by Dr. Slurred speech was then noted, as well as patient noting the weakness was now all over. Dr. recommended patient be evaluated in the ED due to these symptoms - patient agrees, with the agreement that an ambulance would be called to transfer patient also. Patient then noted that the HA was a constant ache and she continued to c/o nausea. Ambulance transfer at 1402.
