At approx. 9:22 AM on 4/2/21, I began having problems swallowing my saliva and felt my throat begin to close. I also began having problems breathing, began coughing and choking, chest pain, and dizziness. The nursing staff at Hospital immediately got a wheelchair and rushed me next door to the ER. The nurses had called ahead so that the ER doctors were ready to assist me. I was given Epinephrine, steroids, nubilizer treatments, Bendryl, IV fluids and oxygen. After some time I began feeling a little better but then the reaction restarted and treatments had to be re-administered several times. It was quite frightening. I also had a severe headache and tightness of the chest . The ER doctor believed that I should be admitted which was done on 4/2/21 and was kept thru 4/4/21.

- An EKG, CT scan, blood tests, regular blood pressure, temperature and oxygen levels were monitored, Covid-19 test, and a lot of blood work on 4/2/21. Additional testing of my blood work was continued on 4/3/21 and 4/4/21.

- Pseudo-tumor cerebri without papilidemia and have a LP shunt with multiple revisions since 2000; recovering from TBI from April 2017 & another one in April 2020 including headaches, vestibular issues, neck pain, visual issues, etc. I also have hypo-thyroid; hi-cholesterol; Bi-polar Disorder I, PTSD, sleep apnea. Had 2 strokes in 1996 and another in 1998. History of seizures. Pulmonary embolisms in 2015. Left knee replacement and other joint issues. Hysterectomy in 2012. Appendectomy in 1965.

- See attached list of allergies: Pfizer Covid-19 Vaccine ? On 4/2/21, I received the first dose of Pfizer covid-19 vaccine at Medical Bldg. @9:09 AM. By 9:22 AM, I had a severe anaphylactic reaction and was taken to ER and admitted. I was hospitalized for 3 days. I will not be able to receive the second dose as advised by the doctors whom treated me. ANTIBIOTICS AZACTAM BACTRIM BETHANECHOL BIAXIN CECLOR CEPHALOSPORINS CINOBAC CIPRO DOXYCYCLINE ERYTHROMYCIN FLAGYL GENTAMICIN IM KEFLEX MACRODANTIN ROCEPHIN SULFA DRUGS TETRACYCLINE TOBRAMYCIN TRIMETHOPRIM There is a large list of antibiotics that were tried/used in which I had a moderate to severe allergic reaction. Unfortunately, I do not have a list of all of the antibiotics that I?ve had allergic reactions to. Many were given at hospitals/doctors? offices/clinics, etc. PAIN MEDICATION ALLERGIES: (Severe reactions including respiratory problems) CODEINE FLORINEF HYDROCODONE MS CONTIN OXYCODONE OXYCONTIN PERCOCET TEGRETOL TRAMADOL/ULTRAM VICODIN The only pain medications that I can take are: Tylenol, Advil, & Dilaudid. PSYCHIATRIC DRUGS: MANY OF THE DRUGS ON LIST BELOW CAUSED AN ANAPHALACTIC REACTION WHILE OTHERS CAUSE SYMPTOMS AS NOTED BUT THERE MAY BE DRUGS THAT ARE NOT LISTED/MISSED. ABILIFY ? dry mouth, rash on hands, under arms, stomach, tongue and body movements, tremors, and head movements. Went to ER with anaphylactic reaction. AMITRIPTYLINE ? Rash, severe allergic reaction. ARICEPT ? severe allergic reaction, choking, light-headed, trouble breathing and throat partially swollen shut. Taken to hospital-given prednisone, fluids, and oxygen. ASENDIN - Tardive dyskinesia, mood swings, anger/irritability, problem concentrating. ATIVAN ? Rash, allergic reaction. BUSPAR ? Lightheadness, dry mouth, inability to sleep, overstimulated, hypo-mania. CALAN/VERAPAMIL ? gained excess weight, swelling of legs and ankles, trouble breathing. CELEXA ? Rash, allergic reaction. COGENTIN/BENZTROPINE MESYLATE ? Caused extreme mood swings, dry mouth, blurred vision, dizzy, dramatically increased heart rate, and tremors. CYMBALTA ? Caused confusion, dizziness, tremors, erratic behavior, rash, and trouble breathing. DEPAKENE ? Severe allergic reaction, hives, unable to breathe, taken to ER for treatment. DEPAKOTE (ALL FORMS) - Used off and on for 14 years, it never helped. At 3000 mg, I developed a rash, tongue swelled, throat swelling and trouble breathing. DESIPRAMINE ? Caused severe rash, problems with balance and coordination, very sedating. EFFEXOR/EFFEXOR ER ? Rash, allergic reaction-severe, caused extreme mood swings. EDRONAX/REBOXETINE ? Purchased and helped for many months a while but caused an extreme dry mouth. Never approved by FDA. FLORINEF ? Severe mood swings, depression worsened, inability to sleep. GABITRIL/TIAGABINE ? Caused vaginal tearing with bleeding and passing of clots, small tears in urethra with blood in urine. It was determined by ER and psychiatrist that this is a rare side effect from this medication. GEODON ? rash, hives, tremors, tongue movements, and body movements. HALDOL ? helped control panic attacks for a while but developed low BP, tongue movements, had uncontrollable body movements (like tremors), hives. HYDROXYZINE ? Caused mixed state of moods. INTUNIV ? Cause terrible tremors, headaches, hypo-mania, and dry mouth. ISRADIPINE - Allergic reaction-severe. Did not help mood. LAMICTAL - higher doses causes a rash and mood becomes more unstable and erratic. I can take low doses and it helps my Bi-Polar disorder I. The generic Lamotrigine causes hypo-mania and does not keep Bi-polar disorder stable. LEXAPRO/ESCITALOPRAM - Caused mixed state of moods. LIBRIUM - Allergic reaction-severe. LITHIUM ? Gained 140+ lbs. while on this medication, did not help my bi-polar disorder because I was also on antidepressants?. Stopped medication due to a bad rash & itching. Tried Lithium again in 2019 which caused hives. LYRICA ? Did not help mood - Psychiatrist stated that drug rep. reported that Lyrica was somewhat helpful in treating depression/anxiety. ER visit due to tongue & throat swelling. MELLARIL ? Caused nausea, vomiting, became dehydrated, irritable, couldn?t sleep, racing thoughts, confusion and hallucinations. Hospitalized to treat symptoms and stopped medication. MOBAN ? Hives, difficulty breathing, difficulty with sleeping including irritability and mood swings. NARDIL ? Severe allergic reaction-difficulty breathing, suicidal feelings, weight gain, edema. NORTRIPTYLINE/PAMELOR? Caused hypo-manic symptoms. PARNATE ? Severe allergic reaction, hives, swelling of tongue and throat, trouble breathing. Sent to ER for treatment. PROTRIPTYLINE ? Caused problem sleeping, dry mouth and unstable mood. PROZAC ? Caused severe mood swings. RAZADYNE - Made mood worse. REMERON ? Caused severe mood swings, rapid heart rate, inability to sleep, irritability. RESTORIL/TEMAZEPAM - Extreme mood fluctuations, hospitalized due to suicidality. REXULTI ? Didn?t help mood and caused tremors, mouth movements and head shaking. RITALIN - Extreme mood swings, hypo-mania and mania. SAME ? Caused mania. SAPHRIS ? Caused hives, trouble breathing, increased blood sugar level. Did not help mood. SEROQUEL ? Hives, severe tardive dyskinesia. I have taken this in 2019 in a low dose to help with sleep but it caused some shaking of body and head tremors. SERZONE ? Caused mood swings, constipation, agitation, increase in headache, extreme weight gain, and on higher doses resulted in hives and problems with breathing. THIOTHIXENE/NAVANE ? Problems with breathing and hives. THORAZINE TOPAMAX (ALL FORMS) ? severe rash, hives, mouth movements, throat closed-up. Used Epi-pen and went to ER. TRAZODONE/Desyrel ? low dose left me feeling sedated, high doses caused hypo-mania, tongue and throat swollen, Sent to ER. TRILAFON ? While on this medication, I developed uncontrollable body movements. It took a very long time for this to decrease. TRILEPTAL/OXCARBAZEPINE ? Bad rash with hives, tightness in chest, difficulty breathing, increase in anxiety. Went to ER for severe reaction and placed on Prednisone, IV fluids, oxygen, nebulizer treatments, and hospitalized for 3 days. VALIUM ? Did not help anxiety. VRAYLAR ? Caused mild seizures/tremors, terrible muscle cramps in lower extremities (thighs, legs, ankles, feet, and toes), problems sleeping, mild shaking of head, trembling of hands, and swelling of ankles, feet and hands. WELLBUTRIN & WELBUTRIN SR ? Dry mouth, severe rash, and mouth swelled up. Used Epi-Pen, went to ER and placed on Prednisone, fluids, and Zantac. XANAX ? Did not help anxiety. ZYPREXA ? Caused tremors, slurred speech, and dry mouth. The list of drug allergies represents the notes from doctors, hospitals, my husband?s and my records. I am unable to find all specifics of every allergic reaction but this is everything that has been accumulated over the years. SLEEPING MEDICATIONS THAT DO NOT WORK: AMBIEN LUNESTA RESTORIL SONATA MELATONIN ? Caused hypo-mania. Meds that were unsuccessful in treatment of Pseudo-tumor cerebri w/o papilledema: DIAMOX ? Used to treat Pseudo-tumor cerebri without papilledema. Also known as idiopathic intracranial hypertension without papilledema. Didn?t help headache! LASIX ? Used to treat Pseudo-tumor cerebri without papilledema. Also known as idiopathic intracranial hypertension without papilledema. Didn?t help headache! Other medications taken that caused various OTHER side effects: Clonidine - Caused BP to drop excessively. I passed-out in yard which caused my LP shunt to break. This medication was tried many times for PTSD symptoms & always caused by BP to drop excessively. Cyclobenzaprine ? Prescribed by headache specialist to help with headache but didn?t help and caused minor allergic reaction (hives, swelling of hands & face). Diozide ? Severe allergic reaction particularly on face, BP very low, my doctor called 911 and I was taken to ER. Fentanyl Patch ? Resulted in a bad rash. Imitrex ? Injections to treat headache ? didn?t help, caused nausea and lightheadness. Inderal/Propranolol ? Used to treat headache, made extremely sick in stomach with nausea, trouble breathing, seemed to make asthma worse. Methocarbamol ? Caused inability to sleep, mood unstable. Orap ? Caused dry mouth, mouth movements, blurred vision, tremors, and drowsy. Periactin ? Caused dry mouth, excessive drowsiness, and didn?t help nightmares. Pimozide ? tremors of upper extremities, muscle spasms, rash, hives, breathing problems, mood swings. Placed on Benadryl and Prednisone. Prednisone ? Causes extreme mania but have had to use to control serious medication allergies. Vistaril ?didn?t help symptoms and caused a rash. Zarontin ? Caused increase in mouth movements, light-headed, drowsy. I cannot take over the counter cold/flu/cough medications because they cause hypo-mania/mania. I can take Delsym and Tessalon Perles for cough. IMPORTANT: Steroids cause hypo-mania/mania. FOOD ALLERGIES AVOCADO MANGO I can have some caffeine and chocolate but only in moderation. Can cause migraines. I have diverticulitis/diverticulosis and cannot eat foods with nuts or seeds. ENVIRONMENTAL ALLERGIES DUST MOLD DOGS CATS ANY ANIMALS WITH HAIR!
