Pfizer BioNTech COVID-19 Vaccine EUA: COVID-19 case resulting in Hospitalization / Death. Patient received Pfizer Vaccines on 3/10/2021 and 3/31/201. Presented to ED with complaints of weakness, diarrhea, and altered mental status. Admitted for hepatic encephalopathy and COVID-19 infection. Treated with dexamethasone, remdesivir, antibiotics, ascorbic acid, zinc, vitamin D, paralytics, and pressors. Developed pulmonary edema, respiratory status decompensated requiring mechanical ventilation with proning. On 1/10/2022, patient was made DNAR/COT and hemodialysis was initiated. On 1/11/2022, the patient went in to PEA arrest and expired..

The reporter is the patient. A 31-year-old male patient received bnt162b2 (BNT162B2), administered in arm left, administration date 28Apr2021 11:45 (Lot number: EN206) at the age of 31 years as dose 2, single for covid-19 immunisation. Relevant medical history included: "Known allergies: Penicillin" (unspecified if ongoing). Patient did not have COVID prior vaccination. Prior to vaccination, the patient was not diagnosed with COVID-19. The patient's concomitant medications were not reported. Past drug history included: Advair, reaction: "Known allergies: Advair"; Codeine, reaction: "Known allergies: Codeine". 03/04/21 No symptoms until 9:15 a.m. (approximately 30 mins after vaccination), began feeling faint, weak, rapid pulse, heart skipping, numbness and tingling in fingers and toes, ringing and heat in ears, rash/redness on chest, neck and ears, intermittently "seeing stars", extremely dry mouth, hoarseness in voice and dry cough when speaking. Made my way to my sister's dental office nearby, where approximately 1 hour later, she gave me 50 mg of Benadryl and 99% Oxygen 2L/min via nasal cannula. BP 131/82, pulse 81, blood sugar 126 ml/dl. When not resolved after approximately 1.5 hours, I proceeded to the hospital Emergency Room. They did an EKG (normal), gave me famotidine and a steroid and observed me for a couple of hours. Most symptoms had subsided and was released at approximately 2:30 or 3:00 p.m.

During observation period after receiving vaccine, client became nauseous, short of breath, and itchy. Our staff moved patient to lay down on a mat, shortly after that pt was no longer oriented x2. Client quickly became oriented x3 after that but began complaining of intermittent chest pain. SP02 was 94%, HR 94-97, client still short of breath and diaphoretic. Gave Bendadryl around 5:15pm with resolution of itchiness. Patient rated chest pain as 10/10 specifically on right side and his back. Complained of metalic taste in mouth. Heart rate up to 114 and SP02 sitting around 96%. Ambulance arrived at 5:25pm and took pt to hospital.

The following information was reported: ABDOMINAL PAIN (non-serious) with onset 15Mar2021 16:00, outcome "recovered", described as "severe abdominal cramps"; NAUSEA (non-serious) with onset 15Mar2021 16:00, outcome "recovered", described as "nausea"; VOMITING (non-serious) with onset 15Mar2021 16:00, outcome "recovered", described as "vomiting"; DIARRHOEA (non-serious) with onset 15Mar2021 16:00, outcome "recovered", described as "diarrhea". The events "severe abdominal cramps", "nausea", "vomiting" and "diarrhea" were evaluated at the emergency room visit. A 46-years-old (non-pregnant) female patient received first dose of bnt162b2 (PFIZER-BIONTECH COVID-19 VACCINE, Formulation: Solution for injection, Lot number EN206, Expiry date: Unknown) via an unspecified route of administration, administered in Arm Left on 04Mar2021, at 03:30 (age at vaccination: 46-years-old) as dose 1, single for COVID-19 immunisation. Medical history included hypertension, hyperlipidaemia and Perimenopause from an unknown date and unknown if ongoing. Concomitant medication included lisinopril 10mg; ezetimibe (ZETIA) 10mg; and boron, calcium, cimicifuga racemosa extract, folic acid, glycine max extract, magnolia officinalis, nicotinic acid, pyridoxine hydrochloride, riboflavin, selenium, thiamine, tocopherol, vitamin b12 nos (ESTROVEN) all taken for an unspecified indication, start and stop date were not reported. patient previously took Amoxycillin and experienced Drug hypersensitivity. Patient was not pregnant at the time of vaccination. Patient did not receive any other vaccines within 4 weeks prior. The patient was not tested positive for covid 19 prior to vaccination and had not been tested post vaccination. On 06Mar2021, 2 days after vaccination the patient experienced red spots developed over my chest, stomach, back, abdomen and legs, then a spot on left wrist the next day, then over the next two weeks red spots developed over chest, stomach, back, abdomen and legs. The spots are dime sized or smaller, and don't really itch, but continue to spread over the last 2 and a half weeks. Patient visited Doctor or other healthcare professional office/clinic visit. Treatment received for the adverse event was Hydroxyzine 10 mg, it hasnt helped at the time of reporting. The outcome of the events were not recovered. No follow-up attempts are needed, no further information is expected

Past medical history included low thyroid and high cholesterol from an unspecified date. Concomitant medications included simvastatin (MANUFACTURER UNKNOWN), levothyroxine (MANUFACTURER UNKNOWN) and venlafaxine (MANUFACTURER UNKNOWN) from an unspecified date for unknown indication received within 2 weeks of vaccination. I had to run for the bathroom with sudden-onset diarrhea. No cramps, not much pain to speak of--just a little. My intestines have cleaned themselves OUT beginning last night and continuing to today (noon). I will be feeling nothing and then suddenly--have to RUN for the bathroom, and don't be late.

Flushing Pruritus

Symptomtext

15min after receiving vaccine, client started complaining of itching and flushing. The itching was located on pt's back. 25mg of Benadryl was given at this time. Pt denies wheezing or difficulty breathing. About 8 min after original complaint client stated that back was less itchy, still complaining of itchiness on face. No rash was noted by observation RN. Observation RN determined it was safe for client to leave at the end of observation time.

Started with Allergies that developed into a sinus infection and bronchitis. Was put on Amoxicillin and BEnzonatate for cough. The sinus infection seems to have cleared up but still have an ongoing cough and congestion from Allergies.
