Narrative: 75 y/o male with dementia presented to ER on 1/25/22 with weakness, falls, and worsening confusion. Increased cough and congestion. Darker urine and dry heaves but no vomiting earlier in the day. Tested positive for COVID. Treated with remdesivir and dexamethasone. Patient died on 2/3/2022. Cause of death likely due to progression of COVID vs treatment medications. PMH of hypertension, dementia, diabetes, atrial fibrillation on apixaban, aortic dissection, and hyperlipidemia.

Spouse indicated her husband (deceased) complained of neck pain, head fullness, and "I just don't feel right" within the first 30 minutes post vaccine. She indicted he laid down at home, then 3 hours later got up and went into the bathroom. Minutes later she found him lifeless on the floor. EMS alerted (911). Patient was provided CPR per EMS, taken to the local hospital, where he was unsuccessfully "coded" and pronounced dead. ECG and cardiac enzyme were consistent with STEMI. Due to the patient's having never having had any signs or symptoms of CAD, COPD, nor diagnoses of cardiac risk beyond mild HTN, age, gender, and elevated lipids, --- nor his having any family history of CAD --- and due to his first post vaccine symptoms have begun only 30 minutes after its receipt, and his sudden death taking place only four hours post vaccine, --- and that his family says he was feeling fine before the vaccine and at the time of its receipt --- I ordered an autopsy. I also had a CRP lab run off of ER labs. This was elevated at 50. I asked that the state coroner / MD carrying out the autopsy to look for sign of inflammation in the heart / coronary arteries. Several weeks later -- the state coroner advised me he did indeed find "inflammatory cells" in the endothelium of the coronary vessels, "not a common finding of MIs". He also found coronary artery disease and a rupture of the myocardium, consistent with MI.

I was disoriented. My speech wasn't clear. My right eye (maybe or left) was drooping. My balance was off. Those lasted a minute or two. I was walking at the time, but when I sat back down I got really tired, so the total episode was five minutes. It was a one time event. Had tried to make an appt with neurologist but it didn't work out. Several weeks after that I went to to urgent care and they recommended I go to the ER. I had a UTI, they said also that with the symptoms listed above it was a possible TIA. They gave me antibiotic, it was a strong one, five day course. I went to neurologist finally after being on the waitlist: That was July. Neurologist, the differential diagnosis is TIA. Silent migraine, EEG, medicine for seizures. See him again November 1st. It's the generic form of Keppra. They referred me also to get my heart checked out, I had a heart monitor, everything seemed to be checking out okay with that.

About 15 hours after receiving the vaccine I woke up to having chills, uncontrollable shaking, fever, headache, muscle and joint pain that was so severe I could hardly walk or stand. I then started to vomit every 15 minutes for the next two days and could not stop. I called my doctors office and was told to call 911 immediately and go to the emergency room.

Tongue tingling Narrative: On 3/26/2021 at 1233, patient received first dose of Pfizer vaccine. At 12:45 patient reported that his tongue and throat were tingling. Feels kind of like my tongue is getting a little fat. Vitals at 1245 BP 166/70, HR 77, O2 97%, provided ice pack. At 1301 vitals BP 119/76, HR 70,O296% top of tongue tingling. At 1318 BP 149/82 HR 88, O2 96% all tingling stopped. No other complications reported. Patient left to go home at 1318.

8.5 hours after vaccine injection, I suddenly began experiencing fatigue and extreme joint and muscle soreness. Initially I noticed my knees hurting with radiating pain up into my thighs. My head hurt, particularly at the back of my skull, near my neck. My neck muscles were sore, as were BOTH shoulders - my right shoulder (injection site) was only slightly sorer than my left shoulder. I called a 24/7 nurse associated with insurance company to ask if ibuprofen or tylenol were acceptable treatments for my symptoms. She told me I could take either and alternate if I felt I needed to maximize relief. I noticed the joins in my toes and fingers were hurting. It was frightening to experience tightness in my chest and back muscles. I never felt like my lungs were compromised, there was just a heaviness as the muscles seemed constricted. I felt a slight rise in my heart rate and experienced a few palpitations mostly because I kept pondering how much worse my symptoms could get. I was a little cold, so I decided to go to bed after drinking extra water (also a suggestion from the nurse). I slept for a few hours 10:00 PM - 4:00 AM then got up and called an advise nurse to report that my joint and muscle pain were not subsiding. She encouraged water and more painkiller. As the day wore on, I felt less achy and more shaky/jittery. My thinking was somewhat unclear - I felt a little dizzy, out of balance, off-kilter and brain "foggy." As I write this 36 hours after my injection, my symptoms are subsiding. Where I felt a little chilled during our cool and rainy afternoon, I now feel a wave of heat sort of traveling through my body. It isn't a fever - just an odd sensation that something is still going on.

The patient took concomitant medications. Vaccination history included: Bnt162b2 (Lot number: Pfizer EN6203, Administration time: 05:00 PM, Vaccine location: Left arm, Dose number: 1), administration date: 08Mar2021, when the patient was 61 years old, for COVID-19 immunization. Additional Information: The reporter stated that within 24-48 hours he had, muscle and body pain, nausea, very tired & lack energy, neck tightness and sever pain. Exercise tolerance was very poor and immediately makes all the symptoms much worse. The worst was over in about 48 hours from on set. But then he had not recovered any more 2 and half weeks. The reporter could not do any of the physical things he did before. If he did not move, then stiffness set in and the first movements were very painful till he got about 30 steps or so. The reporter further stated that he was not tested COVID prior and post vaccination. Other vaccine in four weeks was not administered to the patient. No follow-up attempts are possible. No further information is expected.

My lymph were swollen the next day. For two weeks I had the same problem, then I went to the doctor and they gave me medication. They thought it was because of the medication. A 10 years-old female patient received first dose of BNT162B2 (PFIZER-BIONTECH COVID-19 VACCINE, Solution for injection, Lot Number: EN2613 and expiration date: not reported), via an unspecified route of administration, on 02Apr2021 (age at vaccination was 10 Years) as dose 1, single for COVID-19 immunization. Patient Medical History included ongoing allergy from an unknown date. Concomitant medication included loratadine, pseudoephedrine sulfate (CLARITIN-D) taken for an allergy, start and stop date were not reported. On an unspecified date, the patient experienced 10 years old daughter had her first dose, legs and ankles were bothering, having problem with her ankles and joint pain. The consumer reported that 10 years old daughter had her first dose and denied scheduled second dose; should have not given because she has not even menstruating. Medical history included high cholesterol, hypothyroidism, kidney stones and had severe COVID in Oct2020, requiring hospitalization. Concomitant medications included levothyroxine sodium (SYNTHROID), sertraline (MANUFACTURER UNKNOWN), potassium citrate (MANUFACTURER UNKNOWN) and trazodone hydrochloride (DEPRAX) for unspecified indication from an unknown date. Prior to the vaccination, the patient was diagnosed with COVID-19. Since the vaccination, the patient had not been tested for COVID-19. On May 10th, I started to feel itchy in my breast area. To took off my clothes and discovered a rash on both breast, on both sides of upper torso. I thought I was have a reaction to peanuts I had eaten. (Note, that I have consumed peanuts before with no issues in the past). By 3:00 pm, The rash & welts was on my stomach, bottom, lower back, groin area, upper thighs, feet, hands. My lips were swolen, My voice changed & my tongue felt swolen. The Doctor at Urgent care gave me a shot of Solumedrol IM -125 mg in my buttock. I instantly felt some relief. I was given a prescription of Prednisone 40 mg for 4 days, told to take Pepcid 20 mg twice a day, Benadryl every 6 hours, Zytrec 10 mg daily for 5 days. I saw my Allergist for an allergy test. She tested me for all nut allergies, eggs. I do not have allergic reaction to nut nor eggs. My Primary care Doctor gave me an Epi pen

pt had pain in her right arm, headache, scratchy and fatigue the day of getting the vaccine. The pain in her arm and the fatigue was getting worse. Her left arm and hand started hurting and she had tightness across her back. Her had pain in the left elbow. The pain was causing her a headache and to be nauseated. On 4/8/2021 she went to Healthcare ER. She had blood test and covid test with negative results. She had an X-Ray of the elbow. She was given medicine for pain and nausea. She was diagnosed w/ Arthritis of the elbow which she has never had before. She was told to FU w/ PCP. She saw her PCP the next week. She was told to continue to take some pain meds she was already prescribed. On 5/4/2021 the patient is still feeling very weak and fatigued all of the time. She still has pain in her both arms. She says the arms are weak and heavy feeling. She is finding it hard to function at the level she was before getting her 2nd dose of the vax because she gets too weak and tired.

Mild chills, headache, body ache all over, fever comes and go, fatigue. Still occurring. What happened as a result of the adverse event? Caller stated she took Ibuprofen this morning, this helps with the chills for a couple of hours but chills comes and go. Note: Since caller denied being or taking any current medications, and no current medical history to report. Recommended her to rest, hydrate and take another ibuprofen to help with the body aches, headache and chills just as long as the last dose taken has been at least 6 hours. Advised caller that these are common side effects that occurs more with the second dose. If her symptoms does not let up recommended her to contact her PCP, or if her symptoms worsen to seek medical help asap. 4/2-Follow up: Patient went to urgent care on 4/2/21 she states she feels a lot better then when the symptoms originally started. Urgent care MD stated what she feels is from the vaccine. Advised to continue the ibuprofen for the fever/body aches. As of 8am on 4/2/21 symptoms have subsided and patient is getting better.

No adverse reaction until 3 months after the last injection. Then I noticed a buzzing in both ears especially loud at night. The buzzing comes and goes, but occasionally becomes quite loud in my right ear (within the last 2 weeks). I had what felt like blockage in both ears for about two months after the buzzing began, but the blockage has gone away. I'm not sure if the tinnitus is a result of the Covid vaccine, but I want to report these symptoms that I am having.

- I saw a doctor on September 10, 2021. He is an ear, nose and throat specialist. He diagnosed tinnitus and gave me some information on possible remedies to manage it. He also sent me for a MRI which came back negative for any illness or brain issues. He provided excellent care and advice, but the tinnitus remains. It is not always present, and I am not sure if it was a result of the vaccine since the onset was 3 months after the vaccine, but I wanted to report it.

Ringing in my ears; This is a spontaneous report from a contactable consumer, the patient. A 57-year-old male patient received the second dose of BNT162b2 (PFIZER BIONTECH COVID-19 mRNA VACCINE, Solution for injection, Lot number- EN2613) via an unspecified route of administration in the left arm on 25Mar2021 at 18:00 (at the age of 57-years-old) as dose 2, single for COVID-19 immunisation. Vaccination facility type was reported as Pharmacy/Drug store. Patient medical history was not reported. The patient had no known allergies to medications, food or other products. The concomitant medications included rosuvastatin calcium (CRESTOR) taking 5mg every other day, with a daily dosage of 2.5 mg/alternate day, Cyanocobalamin (VITAMIN B12) (MANUFACTURER UNKNOWN) and colecalciferol (VITAMIN D3) (MANUFACTURER UNKNOWN) for unknown indications, from unknown start dates and unknown if ongoing. Prior to the vaccination, the patient was not diagnosed with COVID-19. The patient did not receive any other vaccines within four weeks prior to the COVID vaccine. The patient previously received the first dose of BNT162b2 (PFIZER BIONTECH COVID-19 mRNA VACCINE, Solution for injection, Lot number- EN6199) via an unspecified route of administration in the left arm on 04Mar2021 at 19:00 (at the age of 57-years-old) as a single dose for COVID-19 immunisation. On an unknown date in Mar2021 the patient experienced ringing in ears. The event resulted in doctor or other healthcare professional office/clinic visit. Therapeutic measures were taken as a result of ringing in ears and included treatment with ear wax removal from an unknown start date to an unknown stop date. Since the vaccination, the patient had not been tested for COVID-19. The clinical outcome of the event ringing in ears was not recovered at the time of report. No follow-up attempts are needed; No further information is expected. Amendment: This follow-up report is being submitted to amend previously reported information which included Lot number updated from unknown to EN6199 in Historical vaccine notes, Vaccine Facility Information added, 5mg every other day added in narrative for Crestor, Narrative Dose 1 and dose 2 order updated. No follow-up attempts are needed, No further information is expected.

Notice hum in both ears following second phizer covid vaccine injection. May have started earlier but definitely noticed in the month following injection. Tried antihistamine, thinking might be related to allergies or congestion in sinuses/ ears, with no change,. Primary provider checker ears: clear with no fluid. On no medication that lists tinnitus as side effect. Have continued to monitor past 10 months, no change. Have never experienced this in past.

Alopecia areata
Symptomtext
Discovered alopecia areata on scalp behind right ear on May 1, 2021 by hairdresser, about 2" diameter. Think must have started a couple of weeks earlier. Since then, discovered several other smaller areas of alopecia on top back of scalp. Scheduling visit with dermatologist after visit with PCP. Do not think stress is involved. Just wondering IF this might be an adverse immune system response to the vaccine.

A 19-year-old non-pregnant female patient received first dose of BNT162b2 (PFIZER-BIONTECH COVID-19 mRNA VACCINE; Lot number: EN6199), via an unspecified route of administration in the left arm on 08Mar2021 at 13:45 (at the age of 19-year-old) and a second dose of BNT162b2 (PFIZER-BIONTECH COVID-19 mRNA VACCINE; Lot no: EN2613) via an unspecified route of administration in the left arm on 29Mar2021 at 13:45 (at the age of 19-year- old) as a single dose for COVID-19 immunisation. The patient medical history was not reported. Concomitant medications included microgestin (MANUFACTURER UNKNOWN) for unknown indication. Prior to the vaccination, the patient was not diagnosed with COVID-19. Since the vaccination, the patient had not been tested for COVID-19. The patient did not receive any other vaccines within four weeks prior to the COVID-19 vaccine. On 08Mar2021 at 13:45, patient experienced eye twitch in the left eye bottom lid all day and every day. It started the day, when patient got first dose. The patient did not receive any treatment for the reported event. The adverse event did not result in a visit to the doctors or other healthcare professional office/clinic visit, and emergency room/department or urgent care. The clinical outcome of the event eyelid twitching was not recovered at the time of this report. No follow-up attempts are needed. No further information is expected.

12 hours after receiving the vaccination I was speaking to my husband and my neck and face started to uncontrollably move and spasm. This was painful and concerning. It got violent and much worse over; This is a spontaneous report from a contactable consumer (patient). A 28-year-old female patient started to receive second dose of BNT162B2 (PFIZER-BIONTECH COVID-19 VACCINE, Lot number en2613) on 23Mar2021 11:30 at single dose in right arm for COVID-19 immunisation. Medical history included Covid-19, asthma, gastroparesis, surgeries, gastric pacemaker, allergies to Penicillin, soya. Concomitant medications included dicyclomine, sucralfate, famotidine, and ondansetron. The patient previously received cefalexin monohydrate (KEFLEX) and Amoxicillin and had drug allergy. The patient previously received first dose of BNT162B2 (Lot number: en6202) on 02Mar2021 11:30AM in right arm. 12 hours after receiving the vaccination the patient was speaking to her husband and her neck and face started to uncontrollably move and spasm. This was painful and concerning. It got violent and much worse over the next few minutes, She could not control the movements. She called emergency responders and got an ambulance to the hospital. She was given muscle relaxers and sent home. Over the next two days it got worse and more painful so she went to the ER hospital again and they did a CT and blood work and said it was dystonia. She was given ativan and it stopped the convulsions. Again she was sent home. The dystonia continued to happen all day every day. She had returned to the ER one more time and needed medical intervention to control the awful, life changing adverse reaction. This was a horrible experience. She was completely devastated. The events resulted in Doctor or other healthcare professional office/clinic visit, Emergency room/department or urgent care. The patient had no COVID tested post vaccination. The outcome of convulsions was resolved. The outcome of other events was not resolved.
